Coordinator Support Services Req #: Category: Newly Created Position Status: Full-Time Shift: Day Facility: Newark Beth Israel Medical Center Department: GR-Ped HIV Infectious Project Location: Newark Beth Israel Medical Ctr, 201 LYONS AVENUE, NEWARK, NJ Job Title: Coordinator Support Service Location: Newark Beth Israel Medical Center Req#: Status: Full-Time Shift: Day Department name: GR-Ped HIV Infectious Project Job Overview: A patient care coordinator provides a valuable service to individuals who are hospitalized or receiving ongoing treatment for a variety of illnesses. This person often acts as a liaison between medical staff and patients to ensure that the best possible care is received. These professionals may act as advocates, assist families who are coping with a loved one's illness, and offer supportive and personal attention when other medical professionals are inaccessible. It is also common for a patient care coordinator to recommend special programs and support groups designed to help people deal with the psychological effects of various medical conditions or diseases that accompany a grim prognosis. When patients are dealing with chronic medical conditions or acute diseases, it is often difficult to focus on certain such things as handling insurance issues, completing paperwork, finding specialists, locating physical therapy centers, or navigating the healthcare system in general. In fact, many of these administrative tasks can be downright complicated, even for people who are not suffering from an ailment. A patient care coordinator can streamline many processes and handle some of these time-consuming undertakings. Individuals are then able to focus on healing, rather than paperwork. Qualifications: Required: Bachelor s level professional with a degree in social work, counseling or related field. Two years prior experience working with underserved populations preferred. Excellent Communication skills, both spoken and written. Knowledge of HIV Disease and local community resources preferred. Preferred: Certifications and Licenses Required (if applicable): Scheduling Requirements (non-exempt positions): 8:00 5:00 Days Essential Functions: Manage and maintain a caseload of clients identified as most vulnerable or at risk for challenges with adherence to care. Assists all patients through the healthcare system by acting as a patient advocate and navigator Review provider schedules and individual patient charts and assist the care team in coordinating care for visits and for future healthcare needs. Handle non-appointment related calls from patients. Resolve the reason for the call or route to the appropriate party. Provide an effective communication link between patient and medical staff including relaying messages from providers, gathering information from patient for providers, etc. Support patients and providers in the medication refill and/or Prior Authorization process Coordination / applications for / renewal of entitlements and benefits Navigation through and enrollment in ACA coverage Coordinate with the medical staff to ensure that case management services are provided to patients with complex medical and/or psychosocial problems Assist in coordination of care with pharmacies, insurance companies and other providers in the community Assist with linkage to care for new patients, reengagement for patients lost to follow up, and treatment and care adherence for all FTC patients Manages high risk patient care including management of patients with co-morbidities and treatment adherence issues Facilitates health and disease patient education Leads/supports the improvement of patient access to medications; effectively and promptly manage medication request, while resolving any barriers to obtaining medication. Serves as liaison between pharmacies, specialty pharmacies and insurance companies; assisting in the processing of medication claims. Assist physicians in insurance peer to peer reviews; providing supporting documents and sitting in on conference calls. Manage medication changes; keeps track of changes in patients medication regimes, while assessing any side effects, patient questions, or concerns to sustain compliance with care. Manage medical referrals; assess what medical referrals are made during clinic visits to ensure the appointments are kept and that the referrals are closed out. Establish/ Maintain business rapports with pharmaceutical companies to disseminate current medication education/information to staff and patients. Assist in the patient navigation of Hep B/C patients; initiate prior authorizations for access to treatment. Implement clinic s injectable medication use. Coordinate staff training Initiate/Complete Pharmaceutical Company Patient Assistance Forms Collaborate 30 day lead in medication process with clinical team Coordinate injectable medication shipments from specialty pharmacies to clinic Manage patients target dates and expectant injection dates Demonstrated ability to work effectively in a team environment. Demonstrated problem solving skills in a complex environment. Demonstrated effective interpersonal relationship and customer service skills. Good organizational and time management skills Good working knowledge of local social service resources or skills to acquire and use this knowledge and information expeditiously.
