If you want to have a positive impact in the lives of others come join us!The multi-disciplinary Homeless Outreach Stabilization Team (HOST) will work in the community to provide services which includes substance abuse treatment, medical services, rehabilitative support, case management and peer support services. The purpose of this program is to serve homeless individuals with behavioral health challenges who have demonstrated a combination of continuous service needs and functional impairments who have not shown to benefit from traditional outpatient programs currently available.The Registered Nurse is a member of a multidisciplinary team responsible for providing on-going services to homeless individuals. The Registered Nurse works primarily with clients in need of psychiatric and other medical attention. This position will share case load responsibilities for coordinating medication services for the team. Service locations include emergency homeless shelters, transitional homeless shelters, homeless encampments, transportation hubs, train stations, and other pertinent community location where people may gather. Clinical skills, competence and evaluation for this position is provided by a nurse manager. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Coordinate medical aspects of care, including monitoring and administration of medications in a manner that meets the unique needs of clients.Access the assigned nurse manager for nursing related consultation as needed.Provide education to clients regarding their treatment, medical and mental health condition.Develop and engage clients in health promotion activities such as health and wellness clinics, smoking cessation efforts, and client education programs for disease prevention.On-going assessment of mental status and the positive impact and side effects of current specialized training and consultation to other staff members on medical/mental health issues relating to clients and their care.Participate in psychiatric consultation, supervision, program meetings and in-service trainings; participate in clinical reviews and case conferences for team ply with the agency's clinical accountability policies and procedures; maintain current and complete clinical records; participate in quality assurance ply with applicable program research and evaluation process.Participate as a member of a multi-disciplinary team providing some components of on-going case management services to homeless and formerly homeless adults.Assess clients for range of health care needs and facilitate linkage and on-going connection to relevant urgent, primary and specialty health services.Advocate for client's access to community resources and services, especially primary health care services, ensuring that clients' needs are met and rights maintained; consult and collaborate with community providers to ensure continuity of care.Other duties as assigned.QUALIFIED CANDIDATES WILL HAVE:REQUIRED:RN licensed in the State of WashingtonOne year experience in the direct treatment of individuals with mental illness.PREFERRED:Experience working with the co-occurring disorder populationKNOWLEDGE, SKILLS & ABILITIES:Must have reliable transportation, insurance and an active driver's license and be willing to use personal vehicle in the course of workAbility to communicate and engage effectively with individuals of varied, backgrounds, trauma experiences, cultures, education level and socio-economic or training in physical and psychological assessment, evaluation and psychiatric treatment modalitiesWorking knowledge of psychotropic medications, their uses and side effects.PHYSICAL, SENSORY & ENVIRONMENTAL:The demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Community settings include but are not limited to residences, group/congregate living settings, hospitals, shelters, the streets or anywhere in the environments may not be ideal in terms of cleanliness or being habitable.Daily travel between locations is required.The ability to drive a vehicle, traverse distances up to several city blocks and on various terrains, navigate stairs, get in and out of vehicles frequently is required.Daily use of a computer is required.May be required to lift 10 - 20 pounds.Blood-borne Pathogen Category III - No predictable exposure to blood-borne RANGE (Salary/Exempt): $82,386.88 - $100,370.85**All pay is based on longevity at Frontier Behavioral Health; thus, the starting pay is $82,386.88 annually.
