As a Home Health LVN, you will:- Deliver personalized nursing care to a diverse patient population in the comfort of their homes.- Collaborate with an interdisciplinary team to develop, implement, and evaluate individualized care plans.- Monitor and document patient conditions, including vital signs, medication administration, and wound care.- Educate patients and their families on managing illnesses and maintaining Communicate effectively with physicians, case managers, and other healthcare professionals to ensure the best possible outcomes for your : $45- $55/hour, depending on experience.Requirements:Current Licensed Vocational Nurse (LVN) license.At least one (1) year of experience as an LVN.IV Certification is required.Current CPR driver's license and an insured, reliable vehicle that meets state or organizational requirements.
