Probation Housing Navigator

Home Start Inc. operates programs designed to prevent homelessness, interrupt the cycle of homelessness, and promote self-sufficiency. With rising rates of homelessness in San Diego exacerbated by the pandemic, our programs are designed to ensure homelessness is rare, brief, and non-recurring. Job Summary: The Probation Housing Navigator provides housing navigation support and assistance to clients referred by the San Diego County Probation Department. The Probation Housing Navigator will help clients develop a housing and resource service plan that identifies barriers to success and stability. The main goals of this position are to support clients and help them secure housing, identify opportunities to increase income and career development, and refer clients to resources that reduce recidivism. Duties & Responsibilities: Work closely with the Probation Department to receive, meet, and assess referrals. Conducts initial screenings, immediate needs assessments, and crisis stabilization. Assist clients with developing and executing a housing and resource service plan within designated time periods. Provide resources and referrals that reduce recidivism and lead to stability and self-sufficiency. Maintains client case records, document interactions in the County PCMS system, and assists with reporting requirements. Assists individuals with collecting documents necessary for applying for permanent housing. Identifies housing options, conducts landlord engagement, and supports clients with securing long-term housing solutions. Provides services aligned with evidence-based approaches like housing first model, progressive engagement, crisis stabilization, harm reduction, and trauma-informed care. Attends staff meetings, case conferences, trainings, and supervision meetings. Works collaboratively with co-workers to assure smooth and seamless delivery of comprehensive services to clients. Qualifications: Must be vaccinated from Covid-19. Undergraduate degree in Criminal Justice, Social Services, or related field. Experience may waive education requirements. Minimum of 1 year working or volunteering with underserved populations. Lived experience is a plus for this position. Must submit to a background check via Live Scan (with ongoing alerts), as required by specific programs, funding requirements and licensing regulations. Must have a valid driver's license and a satisfactory driving record. Must have own vehicle. Must demonstrate ability to be culturally sensitive and respect diversity. Ability to work in a fast-paced environment with exceptional problem-solving skills. Physical Demands: Reading and typing on a computer frequently. Sitting for long periods of time at required meetings or in supervision. Daily traveling in car to meet with clients and attend agency/client meetings. Ability to be flexible and work in various locations.
