To learn more about our company, please visit our website at Objective: Chronic Care Coordinator (CCC) works under the Chronic Care Management (CCM) program for the WC Health, Homebase Care or Alliance Mental Health. The CCC will collaborate with all care team members to ensure that patients enrolled in the CCM program receive the attention and care they need to meet their care goals and manage their chronic conditions. The CCC will communicate directly with the patients by phone or the CCM platform. They will provide education, care coordination, follow up and appointment assistance. The CCC works closely with the Medical Assistant to coordinate the care plan needs and follow ups. Essential Functions: Complete individualized patient centered care plans and provide chronic care management services using our CCM software. Manage frequent contact with patients to provide care coordination, support, and manage compliance with the Chronic Care Management program to increase positive outcomes. Maintain ongoing communication to and from Primary Care or Psychiatric Clinician or designee in regard to emergent patient needs and or life-threatening episodes. Document all client communications both verbal and written, in an accurate, efficient, and timely manner Manage up to 500 members. Coordinate with schedulers to make appointments/ Verified if all members have all required appointments. Check referrals. Check on members' medication if needed. Qualify all or as many members as possible for the end of the month. Add all qualified members for Billing. Call members if needed. Created Care Plans/ Update Care Plans as needed. Review members charts as needed. Knowledge, Skills and Abilities: Effective oral written, and phone communication skills Works well independently with strong organization, collaboration and time management skills Knowledge of clinical practices and diagnostic and treatment procedures Ability to use good clinical judgment, critical thinking skills, and to interpret/adapt/apply guidelines and protocols. BILINGUAL speaking is a plus Ability to develop and build trusting relationships with patients and partner practices. The ability to thrive in a fast-paced environment Work independently on assigned tasks. Stay focused on ongoing quality and daily productivity requirements. Must be highly motivated, results-oriented with strong skills in presenting, communicating, organizing, multi-tasking and time management skills Solid problem-solving and consultative skills. Excellent customer service skills demonstrated by positive feedback Position Type: Full-time Education and Experience: Medical Assistant, LPN, RN or someone with 3-5 years of clinical experience (Preferred, not required) Must have experience working with Electronic Health Records (EHR) software as a user Additional Qualifications: Must be able to pass a drug and background check.
