Salary : $50,851.00 Annually Location : Columbus Job Type: Full-Time Regular Job Number: Department: HHSA Division: HHSA - Public Health & Meeting Place Opening Date: 10/24/2024 Closing Date: 11/18/2024 11:59 PM Eastern Description General Statement of Duties This position serves as a key support position within the Polk County Health and Human Services Agency (HHSA) supporting an integrated care model leveraging HHSA and community resources and partnerships. Responsibilities include HHSA staff support, program support, clerical/administrative functions, fiscal awareness, program/service planning and coordination to ensure quality service delivery to the community, and community engagement. Employee must exercise diplomacy and good judgment while working within licensure boundaries. Employee must also exercise tact and courtesy in frequent contact with employees, patients, vendors, and representatives of outside agencies and organizations. Distinguishing Features of the Position Work is performed in accordance with the North Carolina Nurse Practice Act, North Carolina Pharmacy Act, North Carolina Public Health Nurse dispensing rules, agency regulations, program policies, provider orders and evidence based public health nursing practice. Work requires performing a great variety of duties, responsibilities and coordination of work with others. Employee must understand public health concepts, nursing theory, evidence based practice and administrative responsibilities. Complies with public health laws, regulations and policies related to specific programs as they affect individuals, families and population systems, specialized expanded roles and administrative guidelines. Public Health is responsible for preparedness activities in the event of a natural disaster and disease outbreaks. This position is required to participate in preparedness activities and support the team in response to disaster related events; which may include mass vaccination clinics, sheltering and or dispensing medications from the Strategic National Stockpile. Participates in Quality Improvement initiatives, Quality Assurance activities; monitors and reviews quality of assigned programs, particularly as they relate to accreditation documentation. Reviews county specific statistical data and other demographic information to identify service and community needs; Gathers input from the community, clients, staff, and through community partnerships to identify needs. Works with HHSA Program Managers to establish short and long range goals; advises on program activities and short range plans to meet the needs of the programs. Builds and maintains relationships with providers and neighboring counties; Partners with outside resources on strategies to achieve goals and benchmarks. Assists staff with day to day troubleshooting of issues. Collaborates with other Division Supervisors, Units such as WIC, and Contractors to accomplish work plan goals in a cooperative, consistent and efficient manner. Follows all departmental policies and procedures. Performs other related duties as assigned. Examples of Duties Essential Duties and Tasks Responsible for Accreditation Documentation and Coordination. Record keeping and minutes for various agency meetings. Assist with the development and implementation of marketing efforts of the agency. Communicable Disease Case Management and Investigation Support. May perform various nursing duties at the discretion of the Medical Director, Health and Human Services Director, and/or Public Health Nurse. Assist with reconciliation of public health agency and health coalition budget. Special Event Planning and Education initiatives. Vital Records/WIC Back-up. General Record Keeping. Preparedness coordination. H2O Sampling. CMARC/CMHRP Program Monitoring. Liaison with Social and Economic Services to assist and support agency clients. Aid in the development of division-wide program coordination. Other Duties Performs other duties as assigned. Supplemental Information Knowledge, Skills, and Abilities Knowledge of and skill in supporting health and human services functions. Knowledge of obtaining an in-depth nursing history and physical examinations; the application of nursing theory, practice, principles, and techniques employed in the field of public health and related programs including communicable diseases and sexually transmitted diseases (STDs); knowledge in the concepts of human services programs; and the ability to plan and coordinate work. Typical Qualifications Minimum Requirements and Experience Bachelor of Science in Human Services, Nursing or Public Health from an accredited institution and 1 year of related work experience; or an associate's degree in human services, nursing, or related education from an accredited institution and 4 years of related experience. Registered Nurse (RN). Experience in community service work. Program coordination experience. Experience with direct client coaching/support/counseling. Experience working with non-profits and government organizations. Reliable Transportation and a valid driver's license. Proficient computer skills, including Microsoft Office Suite (Word, PowerPoint, and Excel). Able to work nights and weekends. Able to travel to meet with clients and for training. Successful pre-employment drug screen. Successful criminal Background check. Physical Requirements The ability to perform all duties associated with the position including, but not limited to lifting, prolonged standing, driving, sitting, bending, stooping, and operating or performing work with associated tools and machines. Special Requirements A current and unrestricted license to practice as a Registered Nurse in North Carolina by the NC Board of Nursing.
