Responsibilities Position reports to the Executive Director of Integrated Care Management performs a wide variety of duties in Southcoast Physician and/or Health Network Practices providing behavioral health services and care coordination services to Southcoast Health PCP patients or ACO members or individuals with care management delegated to Southcoast by their health plan across the continuum. Duties include coordinating care within an integrated care team, conducting behavioral health assessments and providing behavioral health treatment including individual and family psychotherapy utilizing brief treatment modalities. Serves as a consultant and resource to care management team and practice staff in the management and treatment of behavioral health disorders. Qualifications A Masters Degree from an accredited college or university. Current Massachusetts Licensed Mental Health Counselor (LMHC) is required. Demonstrated ability to perform as part of a multi-disciplinary team. Experience with electronic medical record documentation preferred. Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired. Pay Range USD $29.04 - USD $49.74 /Hr. Rotation Schedule Requirement at least 1 weekend per year
