Discharge Planner - Laurels Recovery Center

Category Counselor - Bachelors Level Description The Laurels Recovery Discharge Planner is responsible for responding to the overall mental health, substance abuse, or co-occurring needs of the individuals in the program. Chosen candidate will be responsible to facilitate educational and treatment groups to individuals in services, follow all program assessments to review case management and referral needs during the course of treatment as well as identify discharge planning needs, meet with all program participants to identify discharge planning needs, assist with case management and referral needs, and develop a comprehensive discharge plan. Candidate will ensure that case management and referral needs are provided during the episode of care, including provision of case management services, completion of referrals, assistance with identification and accessing of community resources, provide transportation to needed resources, service coordination with other providers, assistance with housing, etc.Position Requirements Bachelor's Degree in Human Services field and QMHP preferred with CSAC or CSAC-A preferred; Master's Degree in Human Services field and/or licensure preferred. Candidate must also have extensive knowledge of community and regional resources, experience providing group treatment services, experience working in a residential crisis stabilization program and/or residential detox program preferred, etc. Effective problem solving and decision making skills. Ability to effectively intervene in a crisis situation. Good written and oral communication skills. Knowledge of local human service resources. Ability to use independent judgement and discretion in the assignment of duties. Good organizational and time management skills. Good computer skills and ability to effectively use electronic health record system. Ability to work flexible hours. CMCSB provides mental health, intellectual disabilities and substance abuse treatment services to the residents of Buchanan, Russell, and Tazewell Counties. At the present time, CMCSB operates over twenty locations, to include outpatient offices, mental health recovery programs, group homes and day support programs for intellectually disabled individuals, and a residential alcohol and drug treatment program. In addition, CMCSB has several services that are provided directly in an individual's home or in a supported environment. CMCSB also provides a 24-hour crisis team to assist community agencies and individuals in crisis situations. CMCSB employs approximately 500 employees, which include full-time and part-time employees, as well as individuals receiving services who are employed to work in various day programs.
