One time Home Visit $100/visit.Schedule: 11/5 Westbury, NY and 11/7 Uniondale, NY.Full DescriptionVitalCheck Wellness is seeking a committed and compassionate Patient Care Coordinatorwith experience in phlebotomy to assist patients in making referral appointments and perform skilled specimen collections for employees at private businesses in Uniondale, NY and Westbury, NY.We are looking for someone with excellent interpersonal skills and ability to create a supportive environment. Will consider an LPN, Phlebotomist, or a CNA/ MA and Paramedic with phlebotomy experience (Phlebotomy certificate preferred).Responsibilities:Record patient information, including insurance information, and utilize electronic health records.Discuss and consult health history, diagnosis, and needs with patient on necessary steps for follow up care.Work with insurance companies, assisting with billing and coding. Obtain pre-approval for and maintain relationships with service providers.Maintain professionalism and treat patients with respect.Perform/collect specimens accurately (phlebotomy).Provide excellent customer service to data and create spreadsheets.Requirements:Must have phlebotomy experience. Certified phlebotomist preferred.Proven experience as a patient care coordinator/receptionist/front desk/ or other related medical scheduling experience. Familiarity with EMR system is a plus.Valid license/ service-oriented and have great bedside manner.Strong communication skills; both written and fortable working independently or with little to no supervision.Must be fully vaccinated against COVID-19 including booster shot.MUST bring own laptop/tablet to work.Must be able to reliably commute to Uniondale, NY and Westbury, NY.About VitalCheck Wellness:VitalCheck is a fast-growing healthcare company with presence in US, Asia and Europe.
