GENERAL SUMMARY AND PURPOSE: Under the direction of the Regional Nurse Manager and Nursing Supervisor, or Practice Manager, the Nurse Care Coordinator is responsible for comprehensive coordination of care for patients and their families/caregivers with providers, eliminating barriers to timely care, facilitating interactions through the care continuum, effectively triage phone messages as the primary point of contact for patients and families, conduct pre-visit chart reviews to identify upcoming patient care needs, provide patient education and self-management support, and support quality improvement activities within the practice. The Nurse Care Coordinator works collaboratively with providers and staff to provide care coordination across the health care continuum for all patients within the office setting. SKILLS, KNOWLEDGE, EDUCATION AND EXPERIENCE: Colleague must have an RN license, as defined by their primary work state (Idaho or Oregon), issued prior to hire date. Those colleagues hired before January 1, 2020 must provide licensure or proof of application in process in their secondary state by August 28, 2020. Individuals hired in this role after January 1, 2020, must provide licensure or proof of application in their secondary state by August 28, 2020 or within six months of hire, whichever date is later. Bachelor's of Science in nursing preferred. Three years of direct patient care experience desirable. Knowledge of performance improvement, billing, coding, and documentation requirements highly desired. Knowledge of evidence-based methodologies. Basic Life Support for Healthcare providers certification from either AHA, ARC or the Military Training Network is required at hire in all units, unless currently certified in ACLS and/or PALS as defined in the SAHS Certification Crosswalk. ESSENTIAL FUNCTIONS: Knows, understands, incorporates, and demonstrates the Organization's Mission, Vision, and Values in behaviors, practices, and decisions. Excellent interpersonal, communication and writing skills are essential with the ability to relate and motivate effectively with patients and clinical staff. Maintains quality, safety, and infection prevention strategies. Gathers data, compiles information, and prepares reports. Coordination of Care across the care continuum including; meeting with providers and other clinical staff to review patient care concerns and assists patients in following plan of care, acting as liaison with hospitalized patients and the clinic including follow up with patients by phone shortly after hospital discharge. provides education to patients and families/caregivers; acts as an information resource to other health care professions, students, and community. May include creation of some health education materials. Oversees disease registries including; monitoring patient compliance with treatment play by identifying patients overdue for visits, labs, or referrals and arranges for follow-up services as appropriate, and keeping data base up to date. Conducts pre-visit chart review of patients including identification of needed preventive health maintenance, immunizations, and chronic disease interventions, collaboration and communication with service delivery partners regarding patient status and missing records, and reviewingpatient's pertinent medical history, current diagnosis, provider's plan of care, and allied health consultations to assist in implementing action plans.
