Hours: Exempt, Salaried | Min. 40/ week Location | Department: LifeCircles PACE, Muskegon Overview The LifeCircles clinical team delivers primary care, palliative care, and end-of-life care for patients aged 55 and older. This position is perfect for the healthcare provider who wants to be patient-centered, creative, and collaborative in their approach to care. The successful candidate will join an experienced Interdisciplinary Team and work closely with other Advanced Practice Providers and healthcare professionals in a well-supported, team-based environment. What you'll do Primary role responsibilities of the NP/PA include: Participating in daily interdisciplinary team meetings Completing periodic health assessments Assessing and treating acute needs and illness Advanced Care Planning Patient and Caregiver Education What it takes Current license and certification to practice as a Nurse Practitioner (NP) or Physician Assistant (PA) in the State of Michigan Master's degree in nursing or satisfactory completion of a physician assistant program BLS certification Current DEA registration State of Michigan Controlled Substance License Minimum of 1-year experience with the frail and elderly or long-term care population preferred Why Brio Living Services Make an impact on the lives of older adults!
