Posting Summary Rutgers, The State University of New Jersey, is seeking a Nursing Care Coordinator for NJMS- Ambulatory Care/Infectious Diseases. The primary purpose of the Nursing Care Coordinator position is to coordinate and deliver specialized nursing care services. Working within a healthcare setting, you will be responsible for ensuring that patients receive timely, compassionate, and evidence-based care, while also addressing their unique physical, emotional, and social needs. This role involves collaboration with a multidisciplinary team to optimize patient outcomes and enhance the overall quality of care Among the key duties of this position are the following: Performs triage and medical assessments within scope of practice. Conducts thorough assessments of patients living with and/or at risk for HIV/AIDS to determine their healthcare needs. Coordinates and monitors the delivery of medical treatments, medications, and interventions. Monitors and tracks patient progress, ensuring adherence to prescribed treatment plans. Provides education and counseling to patients and their families regarding HIV prevention, treatment options, and adherence to care plans. Offers counseling and emotional support to patients and their families, addressing psychological and social aspects of living with HIV. For details, please go to Unit Terms of Appointment Staff - 12 month Position Pension Eligibility ABP Qualifications Minimum Education and Experience Graduate of an accredited school of nursing with a license to practice in the State of New Jersey. Registered Nurse Licensure in the State of New Jersey. Prior experience: At least two (2) years of outpatient experience and/or at least 3 years of relevant experience. Two (2) years of clinical nursing experience with familiarity in the use of electronic medical records (EMR). Certifications/Licenses Required Knowledge, Skills, and Abilities Preferred Qualifications Bachelor of Science in Nursing preferred. Experience in Infectious Diseases, HIV/AIDS care is preferred. Equipment Utilized Physical Demands and Work Environment Physical Demands: Position requires standing 2/3 of the time, walking 2/3 of the time, requires sitting 1/3 of the time, use of hands to finger, handle, or feel over 2/3 of the time, reach with hands and arms 1/3 of the time, stoop, kneel, crouch, or crawl 1/3 of the time, talk or hear over 2/3 of the time. Requires lifting of up 2/3 of the time. Occasionally requires ability to push wheeled carts/stretchers more than 100 pounds up to 1/3 of the time.
