Position Overview: Affirmed Home Care is seeking a compassionate and reliable Home Health Aide (HHA) to provide essential care to our clients in their homes. The HHA will assist with daily living activities, ensuring that clients can maintain their independence and dignity while receiving the care they need. This role is ideal for someone who is dedicated to improving the quality of life for others and is passionate about home care. Key Responsibilities: •Provide personal care assistance, including bathing, dressing, grooming, and toileting. •Assist clients with mobility, transfers, and exercises as needed. •Help with meal preparation, feeding, and ensuring clients follow prescribed dietary guidelines. •Perform light housekeeping duties such as laundry, tidying up, and maintaining a safe living environment. •Monitor and record changes in the client's physical or emotional condition, reporting any concerns to the supervising nurse or care coordinator. •Accompany clients to medical appointments, errands, or social activities when necessary. •Offer companionship and emotional support, fostering a warm and nurturing environment. •Adhere to the client's individualized care plan, following all instructions and protocols established by the care team. •Ensure accurate and timely documentation of care provided, including daily logs and progress reports. Qualifications: •Valid Home Health Aide (HHA) certification in the state of New York. •Minimum of 6 months - 1 year experience in home care, elder care, or related fields. •Genuine compassion for the elderly and disabled, with a patient and empathetic approach to caregiving. •Strong communication skills, with the ability to follow directions and report on client care effectively. •Ability to work independently and manage time efficiently while providing high-quality care. •Reliable transportation and a valid driver's license (if required for the role). •Current CPR certification (preferred but not required).
