Job Summary: The Advanced Practice Provider in Psychiatrywill provide advance practice nursing care to employees, patients, families and the community through assessment, planning, prescribing and evaluation interventions in the clinics at Nappi Wellness Institute. Position works collaboratively within the multidisciplinary team to provide care and develops with the patient/family an individualized plan of care and modifies the plan as status changes. Coordinates care with PCP and various outside agencies and obtains referrals into additional programs to support the patient's recovery. Must excel in providing patient centered care to support the patient's recovery.Is a member of the multidisciplinary healthcare team who demonstrates professionalism and leadership within the institution. Exemplifies the Upstate mission, vision and values and acts in accordance with Upstate policies and procedures. Minimum Qualifications: NYS licensed/registered NP specialty area of Psychiatry required. Master's Degree as a Nurse Practitioner & current National Certification consistent to the clinical practice also required.NPI number and be Medicare enrolled with a Medicare Identification Number (or PTAN) (or eligible for Medicare Enrollment within 6 months of hire). CPR-BLS Certification required at the time of appointment. Preferred Qualifications: American Nurses Association certification as a Psychiatric Nurse Practitioner preferred. Prior psychiatryexperience in a Hospital setting. Prior integrated care experience ideal.
