Position Overview: The PRP Counselor will work closely with clients to support their journey toward greater independence, providing personalized rehabilitation services based on individual assessments. You'll help clients develop essential life skills such as personal hygiene, time management, financial literacy, and coping mechanisms. Services are delivered in various environments, ensuring clients can access the support they need, when and where it matters most. Key Responsibilities: Client Support: Provide at least 4 contacts per month for children/adolescents and 6 contacts per month for adults, with each session lasting a minimum of 30 minutes. Collaborate & Coordinate: Work with families, external providers, and community agencies to ensure seamless access to needed services and resources. Rehabilitation Planning: Develop and update individual rehabilitation plans, safety plans, and DLA-20 assessments annually. Documentation: Record all client interactions, interventions, and progress toward goals, ensuring accurate and timely reporting. Holistic Development: Support clients in areas like self-care, social skills, independent living, and wellness promotion based on the 8 Dimensions of Wellness (social, occupational, intellectual, financial, environmental, physical, spiritual, and emotional). Group Facilitation: Lead monthly on-site group meetings to encourage peer support, socialization, and development of coping strategies. Minimum Requirements: Bachelor's degree in a human services field (Preferred) CPR/First Aid Certification Minimum 1 year of experience working with children and adults in a community-based setting Reliable transportation and a valid driver's license Strong computer literacy and familiarity with community resources
