Job Type Full-time Description The Mid-Level practitioner is responsible for providing specialized care to patients with wounds, ensuring their comfort and promoting health. Attends all departmental, organizational, and medical staff (as appropriate to departmental rules and regulations) committee meetings. Requirements QUALIFICATIONS Bachelor's degree and successful completion of accredited Nurse Practitioner program or Bachelor's degree and successful completion of an accredited Physician Assistant program. Current CPR or BLS certification required. Must be supervised by a licensed physician. Must hold and maintain current state licensure as a Physician Assistant or Advance Practice Registered Nurse. If prescriptive authority for controlled substances is delegated, must have a Texas Department of Public Safety Controlled Substances Permit and a Drug Enforcement Agency (DEA) certificate Experience in wound care required. DUTIES AND SCOPE OF PRACTICE - NURSE PRACTITIONER The Texas Board of Nursing recognizes that individual scopes of practice will vary and that what is within the individual scope of practice for one advanced practice registered nurse may not be within the individual scope of practice for another advanced practice registered nurse authorized to practice in the same role and specialty. However, it is important to keep in mind that the Board holds each advanced practice registered nurse accountable for knowing and practicing within his/her professional and individual scope of practice. DUTIES AND SCOPE OF PRACTICE - PHYSICIAN ASSISTANT • The Physician Assistant (PA) may work in any setting consistent with the collaborating physician's areas of practice and function within the PA'S population-focused scope of practice • The PA's scope of practice shall be defined as those functions and procedures for which the PA is qualified by formal education, clinical training, area of certification, and experience to perform as authorized by the Texas Medical Board rules and regulations, and as privileged by the facility POSITION DESCRIPTION - DIRECT PATIENT CARE INCLUDING BUT NOT LIMITED TO: Performs comprehensive assessments of patients with wounds, including venipuncture and diagnostic evaluations. Reviewing and interpreting tests results; taking and recording histories and physical examinations; communication with physicians regarding patient care and follow up. Develop and implement individualized wound care plans based on assessment findings. Provide wound care treatments, including dressing changes, debridement, and application of topical medications. Monitor and evaluate the progress of wound healing. Collaborate with interdisciplinary healthcare team members to coordinate patient care. Educate patients and their families on wound care techniques and prevention strategies. Maintain accurate and up-to-date documentation of patient assessments, interventions and outcomes. Participates in the development, execution and periodic review of the written patient care policies of the wound care clinic. Participates with the physician in weekly review of the patient health records. Physician's orders are reviewed and countersigned weekly. Assures that adequate patient records are maintained, completed and accurately documented, readily assessable and systematically organized; auditing monthly and training. Performs services in accordance with Cogdell Memorial Hospital policies. Participates in appropriate Quality Improvement (QI) for the wound care clinic. Participates as a member of the Professional Advisory Group. Attends Mandatory Meetings held within the department. Adjust work hours/schedule to meet the needs and demands of the department and hospital. Aware of all Emergency Disaster Plans and how to respond if a Disaster occurs. Works effectively with others, demonstrates leadership and teaching ability and the ability to work under pressure. Appreciation of and desire to be part of a health care team dedicated to providing patients optimal care. Performs other duties as assigned REQUIREMENTS • Support CMH's mission, vision, core values and customer service philosophy • Adhere to the CMH Compliance Program • Assures/maintains compliance with all accreditation, statutory and regulatory standards at the local, state, and federal levels • Comply with all organizational policies. • Demonstrate outstanding customer service/advocacy, professionalism and integrity KNOWLEDGE, SKILLS, AND ABILITIES Previous clinical office experience desired; hospital & Clinic experience preferred. Knowledge of medical model and roles of physicians and physician assistants/Nurse Practitioner/Nurse Midwife or Clinical Social Worker. Strong knowledge and experience in wound care management. Ability to make responsible decisions within appropriate scope of practice. Ability to collaborate effectively with physicians on complicated cases. Ability to educate patients, families, and staff in a user-friendly manner. Skill in gathering and analyzing physiological, socioeconomic, and emotional patient data. Skill in developing/revising patient care plan based on patient status. Knowledge of patient assessment techniques including taking medical histories, performing physicals, evaluating health status including state of wellness, and compliance with care recommendations. Strong organizational and interpersonal skills including the ability to communicate effectively, both orally and in writing. Strong reasoning/critical thinking skills. Excellent time management, collaboration, prioritization and problem-solving skills. Proficiency with computer skills including Microsoft Office applications. Willingness to adapt to change and support new technologies as well as policies and procedures in a positive manner PHYSICAL REQUIREMENTS AND ENVIRONMENTAL CONDITIONS • High-stress environment • Exposure to blood and body fluids, communicable diseases, chemicals, radiation, and repetitive motions • Lifting and carrying 51-100lbs • Full range of body motion including handling and lifting patients • Position requires reaching, bending, stooping, and handling objects with hands and/or fingers, talking and/or hearing, and seeing • Mental demands include: alertness, analytic ability, memory, concentration, judgment, initiative, precision, creativity and communication.
