Key Responsibilities: Assesses the resident on a continuum basis to make sure that all areas of the resident's care are met. Participates in the care planning process by initiating and updating the actual care plan, communicating it to the other staff, and by participating as a team member in the actual conference as needed. Administers medications and treatments as ordered by the physician and in accordance with current practice standards and approved nursing techniques. Assigns resident care to nurse aides as per unit procedure. Observes resident, records significant conditions and reactions, and notifies supervisor or Physician of resident's conditions, reaction to drugs, treatments, and significant incidents; updates plan of care to remain appropriate for meeting the resident's needs. Communicates the resident's condition on the Care Plan. Documents resident's condition on nurses record, flow sheets, etc. Assists in all facets of resident care, such as in the area of ADL's and meal service. Assures that the restorative/rehabilitative needs of the resident have been incorporated in the care plan and are performed and documented according to current policy and procedure. Communicates to physician/CRNP's any changes in condition of the resident and takes verbal orders from the physician/CRNP's. Acts as consultant to LPN's and Nurse Aides as needed. Maintains effective interpersonal relationships with residents, families, and visitors. Assures that the established lifting program is followed for residents as identified in their care plan. Using a team approach, assures that unit routines are established, improved, and carried out by the staff. Acts as Unit Manager in their absence; charge nurse on 11-7 as assigned; evening supervisor in their absence. Communicates the mission statement of Baptist Homes and the Nursing Department to other staff, residents, and visitors. Admits and discharges residents as needed. Establishes an environment on the unit which fosters a secure and therapeutic environment for the residents. Participates in various departmental continuous quality improvement activities as related to nursing practice. Assures that the current infection control practices are followed as per standard and procedure. Performs IV Therapy following current practice standards. Responds to life saving situations based upon nursing standards, policies, procedures, and protocol. Reports any unusual incidents following established Baptist Homes' policies. Reports for correction of any unsafe situation within the facility. Participates in various departmental continuous quality improvement activities as related to nursing practice. Demonstrates ability to carry out skills appropriate to patient population served within scope of practice. Demonstrates ability to use equipment according to nursing procedures and as used in the department. Demonstrates ability to carry out procedures following established standards of care. Demonstrates the ability to care for residents as assigned. Demonstrates ability to interpret vital signs, and responds appropriately i.e. notifying attending MD. Demonstrates clinical competence. Demonstrates an appropriate understanding of medications and pharmaceutical aspects associated with drug considerations. Demonstrates professional development/clinical competency in nursing practice. Participates in in-service, projects, or committees as assigned. Maintains all certifications necessary for employment. Demonstrates flexibility, open mindedness, and versatility in adjusting to changing environments and requirements as necessary. Commits to continually improving his/her skills through participations in education opportunities whether offered on the job, within, or outside of the facility, to increase knowledge of work-related issues. Maintains a current knowledge of CCAC, federal, state and other regulations applicable to job. Maintains professional presence when representing Baptist Homes'. Exhibits degree of professionalism consistent with standards in performance, behavior, and appearance. Maintains resident confidentiality and privacy at all times according to Baptist Homes of Western Pennsylvania policies and procedures; uses discretion when discussing resident matters. Demonstrates courteous and cooperative behavior with residents, families, co-workers, subordinates, supervisors, and other department staff. Maintains positive working relationships with Baptist Homes' management and providers; willingly follows directions and requests as appropriate. Exhibits professional standards through appearance and demeanor consistent with Baptist Homes' Society standards; always dresses and grooms appropriately. Demonstrates a thorough knowledge of work assignments; performs assignments in a manner to meet quality and timeliness objectives. Demonstrates knowledge and understanding of Baptist Homes' policies and procedures; seeks clarification or interpretation of questions concerning policies/procedures from appropriate supervisor. Adheres to Baptist Homes' attendance policy by maintaining an adequate absentee rate and provides proper documentation to support absences when appropriate or required. Always complies with applicable safety policies and procedures as prescribed by the organization, as well as government regulations. Plans and completes duties with minimal direction from supervisors. Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Bachelor's degree (B.S.) from four-year college or university; or graduate of an accredited School of Nursing; three to six months related experience and/or training; or equivalent combination of education and experience. Knowledge of federal, state, and JCAHO regulations/standards. Ability to read, analyze, and interpret professional journals, technical procedures, or governmental regulations. Ability to write reports and procedure manuals. Excellent interpersonal relations and leadership skills. Ability to effectively present information and respond to questions from groups of supervisors, residents, families, staff, and the general public. Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations. Current Registered Nurse license; current CPR Certification.
