We are seeking an experienced and dynamic Nurse Manager to lead our virtual team of clinicians focused on delivering high-quality chronic care management services. In this role, you will oversee the development and implementation of care plans, ensure compliance with clinical standards, and foster a collaborative virtual environment that promotes effective patient care and outcomes. Your leadership will be crucial in driving innovative strategies to enhance chronic care management and improve patient engagement.ROLES & RESPONSIBILITIES:Team Leadership: Lead, mentor, and support a diverse team of virtual clinicians, including nurses, nurse practitioners, and health coaches, fostering a culture of collaboration and excellence.Chronic Care Management: Conduct virtual assessments and evaluations of patients with chronic illnesses. Develop and implement individualized care plans in collaboration with interdisciplinary teams. Monitor patient progress and adjust care plans as necessary to achieve health goals.Remote Patient Device Monitoring: Oversee the use of remote monitoring devices (e.g., glucose monitors, blood pressure cuffs, wearable technology). Analyze data from patient devices to identify trends and intervene as needed. Provide timely feedback to patients regarding their device readings and overall health status. Care Coordination: Oversee the development and implementation of individualized care plans for patients with chronic conditions, ensuring that clinical guidelines and best practices are followed.Quality Assurance: Monitor and evaluate the quality of care provided by the team, implementing strategies for continuous improvement and adherence to regulatory standards.Performance Management: Conduct regular performance evaluations, provide constructive feedback, and identify professional development opportunities for team members.Patient Engagement: Develop and implement strategies to enhance patient engagement in chronic care management, including education on self-management techniques and lifestyle modifications.Data Management: Utilize data analytics to track patient outcomes, identify trends, and inform decision-making for program terdisciplinary Collaboration: Collaborate with other healthcare professionals, including primary care providers and specialists, to ensure comprehensive and coordinated care for patients.Policy Development: Contribute to the development and revision of policies and protocols related to chronic care management services, ensuring alignment with organizational goals and best practices.Technology Utilization: Leverage technology to enhance virtual communication and care delivery, ensuring that the team is equipped with the necessary tools and resources. QUALIFICATIONS - EDUCATION, WORK EXPERIENCE, CERTIFICATIONS:Education: Bachelor's degree in Nursing (BSN) requiredLicensure: Current Registered Nurse (RN) license Experience: Minimum of 5 years of nursing experience, with at least 3 years in a leadership or management role, preferably in chronic care management or telehealth.KNOWLEDGE, SKILLS, AND ABILITIES:Strong leadership and team-building skills, with the ability to motivate and inspire a virtual team.
