Seeking a Full-Time Physical Therapist in the Home Health setting for the Fort Worth and surrounding areas! Uncapped visit availability!About our Client:Our client is a privately owned Home Health Agency licensed by the State of Texas. They specialize in high acuity home health services throughout Texas. They offer region-leading compensation including salary, benefits, and productivity bonuses. They are currently hiring for positions in DFW, Austin, Houston, and their surrounding areas. While the base salary starts at $100,000 there is no limit to visits or productivity bonuses.Job Description:Physical Therapist is responsible for comprehensive assessments in the patient's home, including the completion of skilled documentation through KanTime.Evaluate patients and perform appropriate therapy interventionsDevelop a comprehensive plan of care and coordinate multi-disciplinary carePerform prompt clinical documentation and visit notesContinued communication with physician, care team members, patient and familyComfortable working in all aspects of the home health settingImplement and maintain physical therapy protocolsManage patient treatment and outcomes directly, or through PTA supervisionRequirements:Professional appearance and effective communication skillsCurrent Texas Physical Therapist (PT) licenseOne year of physical therapy experienceCurrent Texas driver's licenseCPR certificationExperience with healthcare software (EMREHR) PreferredPrevious Home Health experience is helpful, but not requiredJob Type: Full-TimeSchedule:Monday to FridayWeekendsLicenseCertification:BLS Certification (Required)Physical Therapy License (Required)Work Location:Houston and AustinEMR Used:KanTime- Requirements The productivity scale is designed to assess and track the workload of home health physical therapists based on a weekly total of 28 points. It considers various aspects of a therapist's role, including evaluations, patient visits, OASIS tasks, and discharges. Administrative or supervisory tasks will be awarded productivity points on a case-by-case basis. Physical Therapists Evaluations, Re-Evaluations, and OASIS Recertification's - 1.5 Points Patient Visit - 1 Point OASIS Start of Care and Resumption of Care - 3 Points OASIS Discharges and PT Discipline Discharges - 1.5 Points Non-Billable Discharges - 0.5 Points Total Weekly Full-Time Points Required: 28 Productivity Guidelines: Tracking: Productivity will be tracked weekly, from Monday through Sunday. Productivity Bonus: You may accept and complete as many visits as you would like after your productivity goal has been met. For every point beyond your required goal, you will receive a bonus rate, equivalent to the current company PRN rate: PT visit $75 (per point) Review: Regular performance check-ins will be held to provide feedback and address any areas for improvement. It is expected that you will make every effort to complete your productivity requirement. If you have met your productivity requirement for the week, communicate to the scheduling team if you wish to pass on any incoming new referrals to another therapist. Mileage Reimbursement This policy outlines the guidelines for mileage reimbursement for home health therapists at Adaptive Home Health. It is designed to compensate employees for the use of their personal vehicles while traveling to and from patient homes, ensuring fair and accurate reimbursement for work-related travel. Adaptive Home Health will reimburse therapists for mileage traveled while performing their job duties. Reimbursement is calculated at a rate of $0.62 per mile, with a deduction for an average commute to account for travel between home and the first or last patient visit. Reimbursement Rate: Rate: $0.62 per mile Deduction: 30 miles per day (to account for the average commute)
