Nurse-LPN - Foster Care - Albany Job Details Job Location Albany-New Karner-PCFC - Albany, NY Position Type Full Time Salary Range $27.00 - $27.00 Hourly Travel Percentage Up to 25% Description We are seeking to find a mid-career, compassionate LPN looking for a position outside of a hospital setting who is ready to fulfill a supportive role in client families' lives. This position offers flexibility not found in medical settings and we are open to working with the schedule of ideal candidates if they are able to commit to full-time employment at the agency The ideal candidate excels at maintaining strong professional and personal boundaries while working autonomously and efficiently through utilizing advanced critical thinking skills. This Licensed Practical Nurse will coordinate appropriate medical care, dental care, and mental health services nursing care/services to a youth and adolescent population with emotional, behavioral and physical needs. The LPN will ensure that medical treatment services meet the requirements of the regulating agency and will work onsite reporting to a Registered Nurse as well as off-site in the homes of client families. Responsibilities: Complete the nursing observations report findings to the registered nurse Review lab and test results, reporting critical values to RN and providers Communicate with medical director/providers regarding observation in client's health/behavior Provide client and family education regarding medications and treatments both in-home and on-site Collaborate with staff regarding concerns and behaviors Participate in Interdisciplinary Medication Review meetings Nursing documentation using EHR Update discharge planning needs A valid Driver's License and the willingness and ability to visit client homes for medication counts and assisting in transporting kids to appointments when needed utilizing an insured personal vehicle or company vehicle where applicable. Standard rates for travel reimbursement will apply.
