Creative Solutions of Wisconsin is looking for a full-time Disability Support Specialist with a Social Worker license. Our company headquarters is located in Southeastern Wisconsin, but our team predominantly works remotely to support clients throughout the state. The ideal candidate will excel combining professional experience with the tenacity for research to develop customized support plans. You will serve to manage the client plan ensuring that the allocated benefits are utilized through the best possible programs. Daily workflows include progressing on your assigned cases, ensuring all details are captured and documented with the appropriate systems to meet or exceed your goals. The Disability Support Specialist responsibilities include: Work directly with clients, with various disabilities, and their families through telephone, in person, email and video conferencing. Develop a case specific plan coordinating organizations, resources and the approved hours that best benefits the client. Build reliable and healthy professional relationships with clients that support project progress and trust for the company. Maintain and document authorizations and all supporting details needed to ensure billables are current and team support of client needs are facilitated. Effectively manage your own time in your day-to-day habits to meet daily and weekly billable goals. Update and maintain accurate and professional records in our project management software daily. Track client goals and submit timely reports as required. Stay up to date on DHS and other program policy to provide excellent resource solutions to each client' specific needs. Utilize Creative Solutions process in conjunction with providing custom solution support. Effectively coordinate and communicate, both verbal and written, with large teams of direct supporting professionals and organizations. Provide case specific solutions by "thinking outside the box" to find alternative resources and possible solutions when considering each problem. Inspire others with a positive attitude. Build an authentic connection with respect for different types of people. Required for Success: 3 Year experience working with the disability community Possess and maintain a social worker license Excellent personal time management skills Adaptable to change Likeable/easy to talk to Excellent experience with technology and online solutions including, but not limited to, email, government and agency systems, project management, etc. Strong, professional written communication Excellent follow-up skills and project coordination Thrive in a "Think on your feet" environment Professional experience using a De-escalation strategy Compensation: The full-time Licensed Social Workers are eligible for compensation of $33 - $35 per hour.
