Position Summary: Participates in assessing, planning, and directing, as well as providing actual primary nursing care for an assigned number of residents. Directs basic care provided by nurse aides. Key Responsibilities: Administers medications and treatments as ordered by the physician, and in accordance with current practice standards. Assists in all facets of care to the residents as needed, such as in the area of ADL's, meal services, etc. Assures that the restorative/rehabilitative nursing care plans are performed and documented as regulated and according to current procedure. Performs IV therapy. Assigns resident care to nurse aides as per unit per procedure. Communicates and directs the nurse aides in the provision of care that the nurse aides provide their assigned residents. Using a team approach, assists in establishing individual unit routines. Communicates the mission statement of the facility and the Nursing Department to other staff, residents, and visitors. Documents information on nurse's record, care plan, on flow sheets or other documentation forms as per regulation or procedure. Takes verbal orders from physicians in emergency or urgent situations as regulated by licensure standards. Assists with admissions and discharges as needed. Assures that current infection control practices are followed as per standard and procedure. Maintains effective interpersonal relationships with residents, families, and visitors. Reports any unusual incidents following established Baptist Homes policies Reports need for correction of any unsafe situations within the facility. Ensures that the established lifting program is followed for each resident as identified in their care plan. Participates in various departmental continuous quality improvement activities as related to nursing practice. Demonstrates ability to carry out skills appropriate to patient population served within scope of practice. Demonstrates ability to use equipment according to nursing procedures and as used in the department. Demonstrates ability to carry out procedures following established standards of care. Demonstrates the ability to care for residents as assigned. Demonstrates ability to interpret vital signs, and responds appropriately, reporting abnormalities to the RN and/or provider. Demonstrates clinical competence. Demonstrates an appropriate understanding of medications and pharmaceutical aspects associated with drug considerations. Demonstrates professional development/clinical competency in nursing practice. Participates in in-service, projects, or committees as assigned. Maintains all certifications necessary for employment. Demonstrates flexibility, open mindedness, and versatility in adjusting to changing environments and requirements as necessary. Commits to continually improving his/her skills through participations in education opportunities whether offered on the job, within, or outside of the facility, to increase knowledge of work-related issues. Maintains a current knowledge of CCAC, federal, state and other regulations applicable to job. Maintains professional presence when representing Baptist Homes'. Exhibits degree of professionalism consistent with standards in performance, behavior, and appearance. Maintains resident confidentiality and privacy at all times according to Baptist Homes of Western Pennsylvania policies and procedures; uses discretion when discussing resident matters. Demonstrates courteous and cooperative behavior with residents, families, co-workers, subordinates, supervisors, and other department staff. Maintains positive working relationships with Baptist Homes' management and providers; willingly follows directions and requests as appropriate. Exhibits professional standards through appearance and demeanor consistent with Baptist Homes' standards; always dresses and grooms appropriately. Demonstrates a thorough knowledge of work assignments; performs assignments in a manner to meet quality and timeliness objectives. Demonstrates knowledge and understanding of Baptist Homes' policies and procedures; seeks clarification or interpretation of questions concerning policies/procedures from appropriate supervisor. Adheres to Baptist Homes' attendance policy by maintaining an adequate absentee rate and provides proper documentation to support absences when appropriate or required. Always complies with applicable safety policies and procedures as prescribed by the organization, as well as government regulations. Plans and completes duties with minimal direction from supervisors. Follows up with appropriate staff, residents, or other individuals regarding reported complaints, problems, and concerns, and uses tactful, diplomatic communication techniques in those sensitive and emotional situations. Participates as a part of an integrated work team where resident, family, and staff thoughts and ideas are sought to create a person-centered environment Ensures that all aspects of the community's "Culture of Wellness" are afforded the residents and staff, recognizing the impact this makes on their quality of life. Qualifications: High school diploma or general degree (GED); or one-year related experience and/or training; or equivalent combination of education and experience. Graduate of accredited Licensed Practical School of Nursing required. Knowledge of the regulations related to the long-term setting, i.e., federal state, and JCAHO regulations and standards preferred. Knowledge and understanding of the geriatric resident and their needs essential. Ability to read and interpret documents such as safety rules, operating and maintenance instructions and procedure manuals. Ability to write routine reports. Ability to work as a team member with the nurses and nurse aides of Baptist Homes. Ability to communicate effectively to the residents, families, visitors, and staff. Accuracy in documentation essential to support the reimbursement system. Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
