Posting Identification Number MP41838 JOB DESCRIPTION: The Department of Healthcare and Family Services, Bureau of Long Term Care, is seeking a leader to manage the Home and Community Based Services (HCBS) waiver-Supportive Living Program (SLP). This leader should have a proven track record in the areas of data analytics, policy development and administration, personnel management, and leadership. This position is vital to ensuring compliance with State and Federal regulations which directly affects the health and safety of SLP participants. Thus, enabling participants to live more independently and avoid a more costly placement in an institutional setting. This position is responsible for ensuring the program preserves the participant's privacy and autonomy while promoting health and wellness. Job Description Subject to management approval, serves as the Supportive Living Program Coordinator in planning, directing, and evaluating the Department's Home and Community Based Services (HCBS) Waiver-Supporting Living Program (SLP) Serves as a working supervisor Directs reviews of complaints, incident reports, and annual review results of HCBS Waiver-SLP providers Develops and implements procedures to monitor and control transmittal of HCBS Waiver Supportive Living Program data utilizing the CMS 1915(c) Waiver Application and 372 Report portal Consults with SLP providers to ensure access for HCBS Waiver-SLP services Develops procedures and establishes controls to ensure compliance with Quality Assurance standards for the HCBS waivers-SLP Responds to telephone inquiries from field staff, general public and providers concerning HCBS Waiver-SLP criteria, guidelines, rules and regulations, procedures and interpretations Performs other duties as required or assigned which are reasonably within the scope of the duties enumerated above Minimum Qualifications Requires knowledge, skills, and mental development equivalent to completion of four (4) years of college, preferably with courses in business or public administration Requires prior experience equivalent to three (3) years of progressively responsible administrative experience in a public or business organization Preferred Qualifications Two (2) years of experience working with Home and Community Based Services (HCBS) waiver(s) One (1) year of experience regarding the waiver requirements, policies, rules and regulations surrounding the program and/ or HCBS waivers, including but not limited to performing actions in the CMS 1915(c) Waiver Applications and 372 Reports portal Two (2) years of professional experience developing and managing a supportive agency program Two (2) years of professional experience reviewing and interpreting provider non-compliance reports related to the Supporting Living Program (SLP) to determine immediate health or safety issues to SLP participants or another HCBS Waiver program Two (2) years of professional experience assisting Medical Providers with inquiries related to customer's eligibility and enrollment in Medicaid Two (2) years of experience working with a Medicaid Management Information System (MMIS) One (1) year of experience developing and presenting training to groups of individuals Specialized Skills Two (2) years of experience working with Home and Community Based Services (HCBS) waiver(s) One (1) year of experience regarding the waiver requirements, policies, rules and regulations surrounding the program and/ or HCBS waivers, including but not limited to performing actions in the CMS 1915(c) Waiver Applications and 372 Reports portal Conditions of Employment Requires completion of a background check and self-disclosure of criminal history Requires the ability to travel in the performance of duties with overnight stays as appropriate Requires the ability to utilize and maintain state issued equipment such as a laptop and scanner Overtime is a condition of employment, and you may be requested or required/mandated to work overtime including scheduled, unscheduled, or last-minute overtime.
