Job Description The Community Health Worker (CHW) Manager provides leadership to CHW intervention activities for the company across the state, system integration and strategic utilization of CHWs. The CHW acts as a liaison/connector between the member, care team and the community. As manager service as a thought partner in long and short term company planning; assist with future grant proposals, journal articles etc. Job Summary - The Community Health Worker manager provides direct supervision to assigned staff, continually assessing training and professional development needs. Also provide support to internal key stakeholders, Program Advisor, Program Manager, etc. Lastly assist in hiring and training of staff. Job Duties Provides supervision, oversight, and mentoring to assigned CHWs including setting reasonable expectations. Ensure staff have needed equipment, supplies and understand their project assignment(s) and time allocation(s). Provide continue development to staff in the areas of community outreach, health education, social service navigation, recruitment, home visits, follow up care; coach CHWs on interactions with members and service providers. Consult CHWs on difficult cases. Lead in identifying, planning and or facilitating continuing education opportunities for CHWs, other team members to promote staff growth. Lead in classroom and on-the-job training for CHWs including shadowing and quality improvement activities. Foster teamwork through positive leadership, relationship building, and recognition of staff achievements. Work to support employee engagement and retention through promoting a positive culture that encourages high performance and morale along with a learning mindset. Assist Program Advisor and Program Manager in developing intervention protocols and processes for internal and community based CHW interventions. Work in coordination with project leaders and other team members to implement established quality assurance and process evaluation activities. Including monitoring the intervention process, effectiveness; assigning, monitoring and discussing caseloads and assisting CHWs in setting professional boundaries. Promptly notify project lead(s) of any issues. Monitor performance of assigned projects. Perform other duties as assigned Job Qualifications College degree preferred but not required Minimum of three (3) years of experience in either a volunteer or paid position working in community setting with at risk populations providing a coordination of services is a must. Must have experience leading a team and or managing a health-related program/initiative. Demonstrate ability to lead paraprofessional staff, influence collaboration, skills in team building and program administration. Knowledge of the CHW Model is a plus This a mobile position, meaning that regular travel to different work locations, including homes, offices or other public setting is essential. Will be exposed to weather conditions typical of the location and may be required to stand and or sit for long periods of time. Must be able to prioritize and meet deadlines, detail oriented and exercise good judgement. Above average organizational skills, good listener, and team player. Must be comfortable dealing with individuals from different cultural backgrounds. Bilingual is a plus Must possess and maintain a valid driver license, current automobile insurance and a driving record that meets the guidelines and requirement of the organization. Hours TBD Starting pay: $60,000 to $65,000 plus
