BRIDGES to Success provides short and long-term supportive services to adults with physical and mental disabilities who are living at home, with a family member or out in the community. Thought you would never ask! We are seeking an extraordinary, kind, and compassionate leader who can acts as an advocate, supporter, educator, and caregiver to individuals with disabilities. We believe in our clients and their right to live in the least-restrictive environment. This takes a lot more than just planning your weekly actives, menu, driving routes and outings. There is a lot of FINE print in the laws and statues that must be followed, researched, and understanding legislation verbiage! If you have experience in reading statues, understand 245D licensing and care about making a difference in the lives of others than you are in the right spot! What is a BTS Day Program Manager's day like and what does managing the clients and staff consist of along with the job responsibilities? Develop and/or ensure that there is a service plan for each participant and that service delivery methodologies, and progress are measurable and observable Assure safety, health, and proper supervision of residents according to individual abuse prevention plans and program abuse prevention plans. Organize daily schedule for day program meeting all licensing requests and requirements Demonstrate knowledge and assure all staff understand the Vulnerable Adult policy, and ensure these are being followed. Report improper or suspicious behavior, misconduct of co-workers, abuse, or neglect of residents to the DOS or Minnesota Adult Abuse Reporting Center within 24 hours of becoming aware of the problem. Keep the DOS updated with changes and needs within the program. Maintain professional contact with resident's case managers, guardians, other service providers and extended support teams. Demonstrating positive and supportive behavior with the clients and coworkers. Medication administration, documenting on the services provided daily in the electronic records and computer system. Socializing with residents getting them to enjoy playing games, watching movies, shopping, going for walks, bonfires, or fishing. Provide daily documentation of services provided and monitor direct support staff's documentation for appropriateness and implementation of support plan as written. Conduct Screenings and tours for new referrals and identifies appropriateness of placement. Complete necessary paperwork for counties to reimburse services and follow up on any outstanding balances Schedule and lead program staff meetings/trainings as necessary (at least every month) Always maintain confidentiality. Other duties as assigned. Compensations: $23.00 to $24.00 per hour based on experience Requirements: A baccalaureate degree in a field related to human services, and 1 year FT work experience* OR An associate degree in a field related to human services, and 2 years FT work experience* OR A diploma in a field related to human services from an accredited postsecondary institution, and 3 years FT work experience* OR A minimum of 50 hours of education and training related to human services and disabilities AND 4 years FT work experience* under the supervision of a staff person who meets the qualification identified in 1-3 above. Experience with and understanding of Minnesota Statutes Chapters 245A, 245C, and 245D.
