SUMMARY The purpose of this position is to coordinate discharges with the interdisciplinary team, in addition to the patient's payer source. The Discharge Coordinator will be responsible for ensuring patients have a safe means of transportation to their post discharge destination, whether that be home, SNF, SMHRF, etc. They will also ensure that any after care appointments are scheduled and appointment information is provided to the patient or their designated guardian, power of attorney, or surrogate decision maker, if one is in the picture. The Discharge Coordinator will also play a role in conjunction with the Utilization Review Coordinators to ensure insurance companies are being provided documentation in a timely manner regarding patient discharges, and when documentation is lacking, requesting it from physicians promptly. The Discharge Coordinator will also be responsible for making post discharge calls to all patients receiving a high L.A.C.E. assessment score, to ensure patients are following up with necessary aftercare appointments to help reduce the readmission rate. JOB REQUIREMENTS: Associate's degree in nursing, psychology, social work, counseling, healthcare management, or some college. 1-3 years' experience working with insurance organizations, including by not limited to submitting clinical information, utilization review process, clerical work, and patient services. Excellent interpersonal, verbal, and written communication skills PHYSICAL DEMANDS: While performing the duties of this job, the employee is frequently required to sit, stand and walk as well as occasionally stoop, kneel or crouch. Normal or corrected vision, hearing and ability to speak are necessary. Specific vision abilities required include close vision, sharp eye focus and ability to distinguish colors. There may be periods of stressful, intense concentration with more than average pressure used in a variety of problem solving situations.
