Work Shifts Any Job Description PURPOSE OF POSITION : Under the guidance and direction of the Supervisor. The Head Nurse of the Behavioral Health Unit is responsible for planning, organizing, directing, coordinating, evaluating the quality of care rendered in the unit. As the Head Nurse, she/he has the authority and responsibility of managing the unit so as to guarantee quality care for every patient assigned to the unit and is held accountable by the use of the nursing process. She/he is responsible for the implementation of the Nursing Department Philosophy and objectives of Nursing Care into management of Nursing are on the patient unit. RESPONSIBILITIES: (1) Assesses each patient needs and establishes appropriate plan of care based on Nursing Assessment and Diagnosis. (2) Maintains standards of care by seeing that an individualized care plan for each patient is developed and implemented. (3) Evaluates Nursing Care Plans and the patients response to that intervention; makes modifications as needed. (4) Prepares patient care assignments based on staff competence and priority of patient care utilizing the patient classification system. (5) Analyzes nursing care requirements utilizing the patient classification system. (6) Supervises performance of non-professional personnel. (7) Makes daily rounds to check for accuracy: Charting of medications, Nurses notes, Doctor's order sheets, Patient Care Plans, Narcotic Sheets, 1-2-6 Cart and emergency equipment and Patient Assessment sheets. (8) Assists in developing and modifying the guidelines for Patient/Family Teaching. (9) Make daily rounds with the Physician to participate with the Medical team in developing priorities of Patient Care. (10) Participates in the development and is responsible for assisting in the implementation of unit orientation to all new employees. (11) Develops staffing patterns that provide for quality nursing care on a twenty-four hour (24) per day, seven (7) days a week. (12) Maintains attendance profiles on personnel, which will be reviewed periodically. (13) Counsel staff when indicated and makes recommendations for further corrective action. (14) Communicates effectively with the patient/family, medical, nursing and other staff. (15) Utilizes Nursing Care Plans as guide for rounds, conferences and assignments. (16) Responsible for reporting to the Supervisor any situation or problem on the service which impede the nurse in the performance of their duties. (17) Conducts weekly audits of charts to ensure that Nurses' Notes reflect Nursing and medical Plan of Care. (18) Directs staff and participates in CQI monitoring activities (19) Reports all Accidents/Incidents/Medication Errors to the Supervisor immediately. (20) Assists with answering patient and/or family complaints accurately and submit on (21) Write yearly evaluation on all staff on his/her unit. (22) Conducts on-service patient centered conferences for staff growth and development. (23) Attends committee meetings as assigned. (24) Maintains equipment and supplies on the unit. (25) Creates and maintains a working milieu conductive to producing quality care. (26) Performs any other duties delegated by the Supervisor and/or Deputy Director of Nursing. (27) Coordinates all discharge planning monitoring activity for clinical services provides on-going technical assistance to Interdisciplinary Patient Care team and initiates the Interdisciplinary Discharge Planning Progress record for each patient. Completes referrals to Home Care or other agencies if required. (28) Participates in the Utilization Management Weekly Discharge Planning Meeting and provide update on alternate level of care patients on the unit. (29) Maintains active screener and assessor status, which enables the completion and updates of Patient Review Instrument (PRI) in accordance with the New York State Department of Health Laws. (30) Initiates Department of Health Protocol when necessary (31) With the assistance of the Nursing Supervisor, plans, directs, coordinates and evaluates progress and nursing activities on his/her unit. (32) Weekly audits of Charts. (33) Ensures a safe and secure environment free of hazards to reduce the risk of injury to all customers. (Patients, visitors and staff). (34) Conducts risk assessments to reduce adverse events. (35) Ensures that safety management issues are appropriately addressed. (36) Explains, involves and instructs patients/families concerning the therapeutic regimen and environment of care. (37) Effectively utilizes the electronic as well as the paper medical record Minimum Qualifications 1. A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State; and 2. Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA); and 3.
