Job Description Purpose of Position: The Community Liaison Worker is responsible for providing HIV counseling and Status Neutral Navigation Including but not limited to PrEP and PEP. The range of services for the class of position includes basic case management to assisting supervisory personnel in the planning, organizing, and conducting of disease control efforts in accordance with federal, state, and city guidelines. Duties and Responsibilities: Conducts HIV and Status Neutral navigation and patient education related to HIV, Prevention, and treatment adherence. Approaches and engages the patient in discussion to test for the HIV virus, discuss benefits of PrEP, and navigate accordingly. Navigates community referrals to HIV primary Care for patients who test positive with HIV and are interested in linkage to care Conducts surveillance activities to link patients to the dedicated HIV Primary care services; Visits and communicates with HealthCare professionals (Physicians, Lab Directors, Hospital administration, etc.) to gain their cooperation in providing services and maintaining health guidelines; documents in electronic Medical records (EMR) Prepare required documents associated with grant deliverables as required by state guidelines; Participates in planning and implementation of outreach events to engage new patients in HIV testing and PrEP services. Continuously assess and improve the performance of the department and facility through performance improvement activities by providing resources to employees, gathering benchmarking information, and visiting other external organizations. Exhibits a positive and professional demeanor in and outside of the workplace that furthers the mission, vision, and value of NYC Health and Hospitals Lincoln Medical Center. Meet all compliance standards/ requirements such as the annual employee's health evaluation, five-star training, patient's right to know, etc. Carries out all other duties and responsibilities as assigned. Minimum Qualifications For Appointment to Level I 1. One year of full-time experience in a government agency or community organization in providing community services to the public or in assisting members of the community in obtaining community services; or 2. Completion of an approved six month training program of combined classroom training and on-the-job experience in community liaison work, plus six months of full-time experience as described in 1 above. For Appointment to Level II In addition to meeting the qualification requirements of Assignment Level I, appointment to Level II requires: 1. Two additional years of full-time experience in counseling, community work or community health activities in a government agency or community organization engaged in providing community services to the public, assisting members of the community in obtaining community services or maintaining liaison with schools, community organizations or other government agencies for the purpose of providing assistance and obtaining participation and support for implementation of community or public service programs; or 2. Education and/or experience equivalent to 1 above. Study at an accredited college in sociology, psychology or other behavioral science may be substituted on the basis of 30 semester credits for each year of the experience described above. However, all persons must have at least one year of the full-time experience described above.
