General Summary: The Health and Wellness Nurse will provide comprehensive care to clients by integrating recovery plans and developing personalized wellness plans. The nurse will primarily work in the community, supporting clients in managing psychiatric symptoms, medications, and overall well- being through tailored nutrition and exercise planning. Some office work will be required, including providing coverage for administering injections and/or managing medications. The nurse will work closely with Thrive Behavioral Health's HT Coordinated Care Specialty team and outside healthcare providers to ensure a holistic approach to client care. Essential Responsibilities: Conduct comprehensive nursing assessments, develop care plans, and evaluate treatment effectiveness in direct client care. Primarily work in the community with clients, providing support in real-world settings. Administer medications, including injections, when working in the office, adhering to Thrive Behavioral Health's policies and procedures. Monitor psychiatric symptoms and assist in managing medication regimens to promote client stability. Develop and implement personalized nutrition and exercise plans tailored to individual needs. Support clients during outside provider appointments, ensuring continuity of care and clear communication across healthcare providers. Collaborate with psychiatrists, medical doctors, and a multidisciplinary team to provide integrated, team-based care. Provide consultation and education on mental health, chemical dependency, chronic disease management, and overall wellness to the community. Facilitate education for clients on daily living skills, medication management, and self-care strategies. Assist clients in building support networks to achieve their health and wellness goals. Work collaboratively with the interdisciplinary team, ensuring a unified and coordinated approach to client care. Complete clinical documentation and client records in compliance with Thrive's standards and applicable regulations. Qualifications Education, Experience and Competencies: Bachelor's Degree in Nursing 3 years' experience in mental health setting required. Certifications, Licenses Requirements: Rhode Island registered nursing license required Valid driver's license and automobile insurance which meets Rhode Island minimum standards required, or the ability to obtain necessary transportation in order to perform the responsibilities/tasks of the job Physical Demands The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
