Job Title: Registered Nurse - Case Manager Location: Columbus, OH 43210 Duration: 13 weeks contract Shift: Day 3x12-Hour (07:30 - 16:30) Hours: 36hr/week (12hr/day) Start Dates: November Pay Rate: $45/hr -$50/hr Job Description: Serves as a liaison between hospital and in-home care teams to ensure patients are screened appropriately for inclusion in the program. Facilitates patient screening and enrollment into the Hospital Care at Home program by working with ED and inpatient providers to proactively identify patients. Run reports in the EMR multiple times/day to identify patients that may be eligible, reviewing medical team documentation and documenting findings in the EMR appropriately. Lead multidisciplinary daily huddles to discuss potential patients and available capacity. Communicate action items and ensure team members follow up so patients can be transferred home in a timely manner. Participates in rounding and unit-based huddles as needed to identify potential candidates and provide education about the program. Manages day-to-day work responsibilities for the Hospital Care at Home program to assure optimal patient flow and satisfaction. Verifies patient eligibility for inclusion, including clinical, social and payor requirements are met prior to enrolling patients. Coordinates as needed with internal stakeholders. Introduces program to patient, answers questions, and obtains patient consent. Follows up with patients and families/caregivers (where appropriate) throughout the patient consent and enrollment process. Performs assessments of patients physical, functional, psycho-social needs including home environment, by utilizing screening tools, direct patient and family communication, medical record review, and discussion with other care team members providing services to the patient. Demonstrates knowledge of levels of care including observation and inpatient status and seeks input from Utilization Management as needed. Collaborates with all in-hospital care team members, including providers, nurses, case managers, social workers, and unit clerical assistants, to assist with and ensure a smooth transition from hospital to home. Serves as a clinical point person for the in-home care team and connects them to in-hospital resources and services. Engages other departments to coordinate services as needed. Educates providers, nurses, case managers, social workers and other care team members on the Hospital Care at Home program and its benefits to patients. Displays thorough and accurate documentation of all communication throughout the patient acquisition and enrollment process. Utilizes interpreter services as needed. License & Certification Requirements RN License OH BLS (AHA) Additional License Requirements 5 years clinical experience in an acute care setting, preferably in a med/surg BCLS certification required
