JOB SUMMARY The Lead RN Case Manager supervises a team of RN Case Managers in the inpatient and emergency departments at all NCH campuses. This role involves leadership, team-building, communication, and problem-solving. The Lead RN Case Manager also plays a key role in program development, staff training, and community outreach. ESSENTIAL DUTIES AND RESPONSIBILITIES Support the Director of Case Management and Social Work Manager in implementing and enforcing NCH and departmental policies and procedures. Supervise, orient, and mentor RN Case Managers. Reinforce compliance with department documentation standards of initial comprehensive assessments, readmission assessments, progress, and discharge notes. Ensure comprehensive individualized discharge planning that meets the needs of patients and their families. Identify gaps in services and develop strategies to improve patient care and resource utilization. Participate and promote quality of care initiatives, length of stay, readmissions, and avoidable days. Provide patients/families with resources to meet their medically related social and emotional needs. Ensure safe transition of patients from one care environment to another. Promote patient/family involvement and ensure choice in discharge planning. Collaborate with multi-disciplinary teams, healthcare providers, and community partners to ensure comprehensive patient care and care transition. EDUCATION, EXPERIENCE AND QUALIFICATIONS Minimum of Associate Degree in Nursing required; BSN preferred. Minimum 2 years of supervisory/manager-level experience in hospital Case Management required. Licensed as a Registered Nurse in the state of Florida. Basic Life Support (BLS) certification required from the American Red Cross or American Heart Association. Case Management Certification preferred. Intermediate computer knowledge: Uses Microsoft Word, Excel, Outlook, and Windows.
