Job Title: Registered Nurse I Job Specialty: Home Health Job Duration: 13 weeks Shift: Day 5x8-Hour (08:00 - 17:00) Guaranteed Hours: No Experience: Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice experience License: Must have a valid nursing license in hand Certifications: CPR, BLS Must-Have: - Experience with HomeCare HomeBase (HCHB) EMR and proficiency in OASIS within the last year- Reliable transportation, a current driver's license, and required liability insurance- Willingness to travel up to 75 miles from the home clinic- Open to on-call rotations and weekend work as required Job Description: - Provide, coordinate, and direct home nursing care according to physicians' orders and agency policies.- Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.- Conduct home visits and participate in periodic office meetings.- Manage a caseload of 5-8 patients per day.- Complete documentation within 24 hours of patient appointments.- Achieve weekly productivity expectations of 30 points.- Be open to floating to nearby clinics if patient census is low.- Participate in weekly clinic meetings.- Maintain complete and timely medical records for each Efficiently plan travel routes to optimize time and minimize travel between patient homes.Please ensure that you meet all the listed qualifications and requirements before applying for this role.
