We work every day for a fair and just society where all people have the opportunity to make choices that lead to better lives for themselves and their families.Program Description: The Intensive Mobile Treatment (IMT) Team provides treatment and support services for individuals with frequent interaction with the mental health, substance abuse, criminal justice and homeless services systems and have been poorly served by traditional treatment models. IMT teams stay connected to individuals regardless of their location in shelter, on the street, in housing, hospital or jail. Purpose of Position: The Registered Nurse (RN) works within the collaborative Intensive Mobile Treatment (IMT) model to provide client-centered, field-based nursing care. This role emphasizes holistic health management, addressing both mental and physical health needs using trauma-informed and recovery-oriented approaches. The role requires an RN who can be flexible, solution-oriented, and collaborative across various disciplines.Schedule: Monday through Friday 9:00 a to 5:00 pSalary Range: $99,134.00 - $111,525.00Candidates must have the required qualifications to be considered for the position. The salary offer will be based on the candidate's number of years experience in the exact or comparable position and additional preferred qualifications will be taken into account.Roles, Responsibilities, and Essential Duties1) Medication managementOversee medication management, including delivery, administration, and monitoring of psychiatric medications for up to 27 clients in collaboration with the Psychiatric Nurse Practitioner (PNP).Liaise between the PNP and pharmacies to ensure prompt delivery of medications to the IMT office and/or directly to clients. Maintain proper storage and documentation of onsite client medications including controlled substances and assist other providers in appropriately administering medications prescribed by IMT's PNP.Maintain proper documentation of medications that are delivered and/or administered per program policiesAdminister injectable medications (for example, Long-Acting Injectable Antipsychotics) in community settings2) Nursing servicesAssess psychiatric and medical status of clients; perform medical assessments and health screeningsProvide referrals to primary and specialty care, aid clients in scheduling appointments, escorting clients to appointments, and following up after appointments.Provide psychoeducation to clients and IMT staff on health and wellness Perform or arrange blood draws and urine toxicology screenings as neededVisit clients in residential facilities, on the street, at hospitals, and in other locationsMaintain up-to-date client case records according to program guidelines Provide basic first aid services to clients as neededParticipate in the development of new systems to streamline client health care at IMT3) Participate as an IMT team memberParticipate in the collaborative cross-discipline model of IMTAttend staff meetings, case conferences, and training programs to upgrade skills and knowledgeAttend supervision meetings to review client progress and case management strategiesMaintain up-to-date client case records according to program guidelinesAssist in obtaining medical records, insurance information, and other basic case management tasksProvide medical expertise to team members to aid in holistic understanding of clients with co-occurring conditions.4) Other duties as required or assignedQualifications/Educational RequirementsActive NY State License as a Registered Professional Nurse 2+ years of nursing experience preferredDriver's License strongly preferred Knowledge, Skills and AbilitiesExperience working with people with histories of substance abuse, mental illness, homelessness, and trauma required Understanding of harm reduction techniques is a plusIMT, SMT, ACT, EMS, or similar field experience preferred Fluency in Spanish a plusComputer Skills: To Perform this job successfully, an individual should be Able to work in the Microsoft Office Suite, should especially be proficient in Word, Outlook, and Excel Able to use or learn to use AWARDS database Able to use or learn other electronic databases, programs, and funder-related software as needed such as PSYCKES, HEALTHIX, etc.Physical Requirements/Working ConditionsWalking, climbing stairsField work required; Must be able to travel throughout the 5 boroughsBenefits Eligible- Unionized by Local 74 We are committed to building an inclusive workplace community grounded in respect for differences.
