Shift Schedule: Monday-Friday: 9AM-5PM Hours Per Week: 35 Hybrid Position Program Description The New York Center for Dignity and Support seeks to provide care and services to EmblemHealth members that are struggling with a serious, life-limiting and advanced illness through a holistic (person centered) approach addressing the emotional, physical and spiritual needs. The NYCDS will assist clients and their care partners through the provision of direct services including information and referral, advocacy, care management, individual and care partners support, advance care planning, goals of care conversations, addressing the social determinants of health, and connections to concrete services, palliative and hospice care. Duties and Responsibilities: Working with the Supervisor of Palliative Care Supportive Services, the Social Worker will: Provide clinical consultation in-home, and other appropriate locations in the area of serious and advance illness, end-of-life and palliative care, all in accordance with the standards of the social work profession, agency policies, and requirements of the funding source: Provision of individual and care partner support and care management. Development and implementation of a written care plan and clinical services model based on standardized assessment instruments for client/patient and affiliates. Provision of appropriate information to clients/patients and affiliates regarding challenges presented by illness/conditions and ways of coping and preserving individual functioning and autonomy. Provision of specific information about services available for clients/patients including screening for eligibility, assisting with applications for benefits and entitlements, and arranging for their direct provision Ongoing visits both in person and virtually to client/patient and affiliates to evaluate client/patient functioning, monitoring risk, symptom burden, assess the status of current services, and update and revise care plan. Represent program in community and professional settings: Join various community coalitions and task forces based on agreement with the Sr. Director, funding source and partners. May be assigned other tasks or duties reasonably related to their job responsibilities. Maintain accurate and timely care records and statistics and ensure that documentation meets agency's standards. Assist with maintaining and tracking data, draft reports and participate in site visits as required by funding sources. Requirements: Licensed Master's Degree in Social Work required. LCSW preferred. 2-3 years clinical experience in providing end-of-life, palliative care, bereavement counseling and care-management. Experience providing community outreach and community relations Excellent oral, written, communication and interpersonal skills. Bi-Lingual Spanish preferred Must be computer literate, including ability to learn and use program-specific software An understanding and appreciation of the roles of race, religion, ethnicity, sexual preferences, and individual values as they relate to serving clients and families.
