The primary purpose of this position is to provide clinical support for the processing of home health orders, labs, and physician communication for home health location. Under the supervision of the Administrator, the LVN Care Coordinator (LVN CC) provides clinical coordination and communication with patients, physicians, and other clinicians in accordance with current standards and regulations so that the highest degree of quality care will be maintained at all times. Completes other tasks and projects as assigned by the Manager when necessary. Job Description : EDUCATION Equivalent experience will be accepted in lieu of the required degree or diploma. Other: Graduate from an accredited vocational school of nursing or equivalent. CERTIFICATION & LICENSURE LVN-Licensed Vocational Nurse PREFERRED EXPERIENCE AS TYPICALLY ACQUIRED IN: 1 year clinical experience required; two years clinical experience strongly preferred 1 year of experience in a home care position strongly preferred Experience in coordinating and processing home care orders strongly desired. Experience as a team care coordinator or other similar roles highly desirable. 5 years direct patient care preferred SKILLS AND KNOWLEDGE Must have the clinical knowledge and critical thinking ability to effectively plan and provide coordination of patient care consistent with standards and regulations Must have exceptional interpersonal and customer services skills; must be able to effectively solve unique problems as they arise or identify when to consult the supervisor Must have excellent computer word processing skills with the ability to enter at least 50 WPM Must be able to demonstrate proficiency (after training and introductory period) in efficient use of electronic medical record systems Must be willing and able to learn new computer software with training provided Proficient in Microsoft Office products (Outlook, Word, Excel) Knowledge of home health services and clinical practice, Medicare Conditions of Participation, home health accreditation requirements, reimbursement patterns for Medicare and other payors Must be detail-oriented and excellent in time management Must be able to follow a defined list of tasks and follow the established procedures completing them in a timely fashion Must be able to flex with the increased workflow when census is higher and recognize urgency of each task Must have excellent verbal and written communication skills in all interactions with patients, physicians, and staff, demonstrating excellent customer service Must be able to exercise good judgment in prioritizing workload, working with minimal supervision, and organizing and completing tasks accurately and in a timely manner Ability to maintain harmonious constructive relationships with internal and external customers Job Shift: Days Schedule: Full Time Shift Hours: 8 Days of the Week: Monday - Friday, Variable Weekend Requirements: Rotating Weekends Benefits: Yes Unions: Yes Position Status: Non-Exempt Weekly Hours: 40 Employee Status: Regular Number of Openings: 1 This position may regularly work, store, prepare, receive, unpack, transport, dispose of, or administer drug(s) identified as hazardous, or potentially hazardous, by the National Institute for Occupational Safety and Health (NIOSH) for purposes of USP 800. Pay Range is $37.70 to $50.64 / hour The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate's experience, education, skills, licensure and certifications, department equity, training and organizational needs.
