We are seeking a dedicated Case Manager to join our team and contribute to our signature "Affordable Homes and Services for the Community (HOMES)" program. In this role, you will play a vital part in supporting our residents, primarily families with children, on their journey toward self-sufficiency and stability in their homes. Our Case Managers provide professional, compassionate, and caring support to help residents navigate and overcome obstacles to housing stability. This includes addressing challenges such as unemployment, transportation barriers, lack of childcare, and food insecurity. We serve as accountability partners, walking alongside our families to avail them of opportunities and create goals-based paths to the future. What You Will Accomplish Change lives for the better o Provide supportive case management services to our resident households, including formulation of goals and individualized action plans that guide residents to self-sufficiency o Conduct assessments and periodic reassessments on progress toward goals o Provide service referrals and coordination of service delivery o Administer direct assistance and financial support for residents and their children o Participate in planning and implementing program activities and community events Build meaningful relationships o Build relationships with residents through regular communication and service delivery o Work closely with our case management staff and services team to support resident participation and engagement o Establish positive partnerships with internal and external stakeholders, including representatives of county offices, community organizations, volunteers, and service providers to implement motivating programming for clients Serve as an accountability partner o Set a standard and expectation for a successful tenancy o Support residents with establishing and maintaining a household budget o Reinforce critical actions and comprehensive support and follow-up Couple data-driven results with trusting relationships o Track client progress and ensure proper documentation o Support the Resident Services Director with data collection for grant reporting Experience You May Bring We're seeking a charismatic Case Manager who is passionate about helping families succeed and prosper. The ideal candidate will: Understand the experience of those in at-risk communities and believe in the ability of all people to build better lives Be comfortable forging connections with adults, youth, children, and critical stakeholders to forge partnerships integral to progress, in both virtual and in-person settings Exhibit cultural competency with diverse communities and a willingness to learn to promote connection Have the ability to analyze case information and to reach sound decisions and recommendations Be knowledgeable of, or willing to learn, community resources and service providers in South Alexandria/Fairfax County Have general knowledge of financial literacy, budgeting, and expense tracking Know the principles and practices of case management, including social work values, ethics, and client confidentiality standards Have a Bachelor's in social work, psychology, counseling, sociology, or related fields The following characteristics are a plus: Lived-experience with immigrant, at-risk, housing insecurity, and/or low-income communities Fluency in more than one language Who Should Apply?
