This is a delegating nurse case management position within the DDA community, so you will have to have a Registered Nurse license to apply for this position. Dove Pointe will provide for training time in the CM/DN course. Please note that this is a delegating position so individuals with a CMT and/or CNA license will be working under your supervision. This is a day program position and the hours are Monday through Friday 8am to 4pm. Major holidays off. No on call. Supported by the DON and LPN. Make individual health care plans that include admissions, oversight of medication management, and delegated tasks per nursing care plans. Act as liaison between individual and their families, guardians, advocates, health care personnel, the agency and others as required. Ability to guide, educate, and support direct care staff, CNAs, CMTs, LPNs in the day program based off of nursing care plans. Provide 45 day, 90 day, or annual assessments based on the clients needs and HRST score. Provide acute assessments and treatments for any illnesses or injuries that occur during day program hours. Review electronic Medication Administration Records (MARs), (TARs) on an ongoing monthly basis. Review and update physicians orders (when applicable) every 90 days and as needed. Participate and facilitate execution of yearly PCP process as team member for community clients without outside nursing services. Includes attending PCP meetings and other meetings pertaining to the needs of individuals. Must attend 2 DDA nursing quarterly meetings per year to maintain Delegating Nurse status. Maintain a current Registered Nurse license to practice in MD, CPR and all annual required training's required by Dove Pointe. Practices within the Maryland State Board of Nurse Practice Act using COMAR regulations. Qualifications:Must be able to accurately and timely complete job duties with little supervision. Must be detail oriented. Must be able to work with individuals with all levels of disabilities. Must meet all local health regulations, pass background checks (including fingerprinting), pre-employment physical (and continuous), drug testing (pre-employment, random and reasonable suspicion), and satisfactory employment verification. Must be capable of performing job duties with approved reasonable accommodations or without accommodations. Knowledge of current and established nursing concepts, principles and practices. Knowledge and understanding of Infection Control, Universal Precautions, Incident Reporting, Care Planning and skills to develop and implement nursing standards of practice. Ability to organize and manage multiple priorities. Possess basic knowledge of certification requirements of CNAs and CMTs as set forth by the MBON and DDA MTTP process.
