Life-changing outcomes. CommQuest seeks multiple Outpatient Counselor Assistant roles for Canton, Alliance, and Massillon locations. In this role, you will: Impact your client's quality of life Build valuable relationships Grow professionally Work on a team providing exceptional care Essential Job Functions / Responsibilities * * Adhere to agency policies and procedures. Attend department, staff and other agency meetings and functions as required. Provide counseling services consisting of assessing a client's reasons for engaging services, mutually developing a course of treatment to address the needs as expressed by the client to achieve more effective personal, social, educational and/or career development adjustment. Places clients in the most appropriate yet least restrictive treatment modality with access to appropriate intervention services, with Q.A. chart reviews establishing competency. Orients new clients to the agency's treatment philosophy, hours of operation, service array, clients' rights / responsibilities, grievance procedure, confidentiality regulations, rules and regulations. Diagnosis of mental and / or emotional disorders according to the DSM-V or other current manuals as required. Provide crisis intervention / counseling for individuals to resolve or stabilize the crisis, establishing competency through incident report reviews, training records and progress notes. Provides individual and group counseling, demonstrating competency in individual / group counseling as evidenced by clinical chart reviews, case consultation and supervisory observations. Provides case management to assist clients in obtaining needed services and linkages with referral sources, employers, family members, other treatment providers and to assist in the discharge planning process, with competency evidenced by chart reviews, case consultation and supervisory observations. Performs clinical record keeping which accurately documents the findings, treatment and clinical outcomes on a timely basis for each client served, demonstrating competency by chart reviews, quality assurance reviews and supervisory reviews. Provides client and family education demonstrating competency in clinical progress notes, pre / post test scores and supervisory observation. Meets the CommQuest Services productivity standard guidelines. All other duties as assigned.
