A healthcare network in Connecticut is actively seeking a licensed Nurse Practitioner (NP) and/or Physician Assistant (PA) to join their Primary Care team in Fairfield County. In this role, the Primary Care NP / PA will be responsible for managing a patient panel with a broad array of patient needs. About the Opportunity: Schedule: Monday to Friday Hours: 32 per week Responsibilities The Primary Care NP / PA will be responsible for: Conducting a mix of acute, chronic, and well visits. Treating patients in-office or in testing centers as well as conducting occasional tele-health visits Continuous learning during weekly Clinical Rounds and through other modalities Ongoing collaboration with in-office teammates via daily huddles, as well as with virtual clinical teams Performing in-office procedures, as needed Handling other duites, as needed Qualifications 5+ years of experience as an Advanced Practitioner, including 2+ years of experience in a Primary Care role/setting Bachelor's and/or Master's Degree Completed an accredited Nurse Practitioner (NP) or Physician Assistant (PA) program with a national certification CT Nurse Practitioner (NP) and/or Physician Assistant (PA) license Clinical proficiency in evidence-based Primary Care Solid assessment, clinical, and documentation skills Great interpersonal skills Excellent communication skills (written and verbal) Strong attention to detail Highly organized
