Job Details Job Location Administration - Muskegon, MI Position Type Full Time Education Level 2 Year Degree Travel Percentage Up to 50% Job Category Health Care Description SUMMARY OF JOB RESPONSIBILITIES Full Time, 4 days a week (80 hrs Per Pay) with rotating weekends - 10 hour shifts. This position is responsible for patient referrals and intake activity; liaison with area discharge planners, referrers, payers and patients/families. ESSENTIAL FUNCTIONS AND DUTIES (other duties may be assigned) Maintains the confidentiality of patient and employee information at all times, both at work and off duty, and after the termination of employment. According to the agencies Notice of Privacy Practices and HIPAA regulations. Is knowledgeable and functional in computer applications including database, word processing, spreadsheet, and e-mail system as required for the execution of the job. Knowledge of Hospice concept and philosophy. Facilitates hospice referrals by physicians and hospital discharge planners by liaison activity at area hospitals as needed. Assesses and documents appropriateness for hospice at initial assessment visit. Conducts initial nursing assessment visit and arranges for services to begin. Consults with Hospice Physician and/or clinical manager on anticipate needs for new admissions Provides information on hospice care and programs to referral sources and patients and families Assesses patient/family interest in hospice care and services as needed Determines payer source and coordinates the determination of hospice benefits for the referred patient as needed Coordinates with discharge planners to assure continuity of care and patient admission to the hospice program as needed Medication for patient assessed for need and effectiveness on an ongoing basis using comprehensive physical assessment. Works as a team member and collaborates with appropriate co-workers. Demonstrates proper hand washing, asepsis, bag technique and universal precautions. Completes written admission documentation with patients and families as needed Carries out projects and tasks related to community and public relations as assigned DIRECT REPORT RESPONSIBILITIES This job has no direct reports. Qualifications Current Michigan License as Registered Nurse. BSN preferred with experience in community based care Possesses and maintains a current CPR certification. Current driver's license. Certification in hospital & palliative care preferred.
