Position Summary Assist the RN Case Manager in the plan of care for the patient. Essential Functions To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Assists RN in case management of total plan of care of patient/support persons by coordinating with team members and communicating with IDG in accordance with organizational policies, procedures and regulations that define the scope of practice. Complete, maintain and submit accurate and relevant clinical notes regarding the patient's condition and care given. Record pain/symptom management changes/outcomes as appropriate. Administer medications and treatments as prescribed by the physician. Document nursing care and significant interactions with patients, support persons, and team members. Assess patient/family response to care and participate in follow-up with bereaved families with various types of contact. Manage assigned patients equally, regardless of severity, contagion, or complexity of diagnosis. Participate in on-call rotation Qualifications Licensed Vocational Nurse licensed by the State of Texas Hold a valid driver's license and have a dependable means of transportation 1 year of nursing, hospice, or oncology experience preferred 1 year of LVN experience in an acute care setting is required. Proficient skill using a computer is necessary, Microsoft Office suite experience preferred.
