Job Description About the role: The LVN SNF Liaison plays a crucial role in delivering quality mental health services within SNFs. They build and maintain strong relationships between EMH's clinicians and SNF management teams, ensuring a continuum of care for residents. Liaisons excel in customer service and problem-solving, ensuring that interactions are handled with professionalism and empathy. Key Responsibilities: Support SNF Management and Clinical Team: Build and maintain relationships with SNF management and staff to ensure the highest quality of mental health care. Coordinate and participate in meetings, audits, and surveys to ensure compliance with regulatory standards and Physician Orders. Facilitate in-services, trainings, and psychiatric medication reviews to support both SNF staff and EMH clinicians. Communicate effectively with clinicians and SNF staff, managing patient referrals and eligibility, and ensuring that patients are appropriately added to caseloads. Provide on-site support to clinicians, addressing issues, increasing referrals, and ensuring smooth operations. Act as a liaison between EMH and SNFs, providing daily updates to management and ensuring that all relevant information is communicated to the appropriate parties. Requirements Valid Licensed Vocational Nurse (LVN) credentials. 2-3 years of experience in skilled nursing or hospital settings, with a strong understanding of CMS survey preparation. Medication Reviews: Conducting thorough reviews of patients' medications, especially psychotropic drugs, ensuring accuracy, appropriate dosage, and compliance with prescribed regimens. Survey Preparation: Assisting in preparation for state and federal surveys by ensuring compliance with regulations, gathering necessary documentation, and addressing any areas needing improvement, especially related to patient care and medication management. Familiarity with Ancillary Services: Understanding and coordinating the use of additional services such as mental health, physical therapy, and other specialized care, ensuring integration into the patient's care plan. Collaboration with Physicians and Nurse Practitioners: Working closely with healthcare providers to evaluate patient needs, review care plans, and implement appropriate interventions. The liaison must communicate effectively to ensure coordinated care across disciplines, particularly in addressing behavioral health needs. Strong interpersonal, communication, and organizational skills, with a focus on customer service and time management. Willingness to travel within San Jose/Bay Area, with reliable transportation. Familiarity with electronic health records, medical terminology, and HIPAA regulations.
