Job Type Full-time Description Position Information Full-time (40 hours per week) PTO - 5 weeks accrued per year Health Benefits - Start on day 1 of employment Continuing Medical Education time and funds allocation 401K Match 10 Paid Holidays Off per year Position Summary The Outpatient Therapist (Medication Assisted Therapy Clinic "MAT") provides outpatient adolescent, adult, family, and group therapy, crisis intervention, and health clinic consultations to patients receiving care at the MAT clinic, in accordance with the Heartland Health Services' (HHS) mission, strategic goals, federal and state laws and regulations, performance and outcome objectives, and accreditation standards. Essential Functions Evaluates patients for symptoms, causes, and treatments of substance use/abuse problems and mental health problems. Maintains knowledge of benefits and limitations of urine screening and other toxicological tests; basic understanding of medications used to treat addictive illness/mental health; suicide assessments; and opiate overdose signs and symptoms. Able to recognize signs and symptoms of an opiate overdose and applies accepted criteria for diagnosis of substance use/abuse disorders, screen for psychoactive substance toxicity, intoxication, and withdrawal symptoms. Conducts adolescent, adult, family, and/or group therapy. Provides crisis intervention as needed. Formulates treatment plans and goals and carries out treatment plans. Documents all patient encounters and maintains comprehensive patient files. Counsels and aids individuals requiring social service assistance. Facilitates patient access to community resources, including relevant mental health services; assists patient to make contact with social support networks. Evaluates community organizations to gain knowledge of resources available to patients. Maintains a positive working relationship with department. Communicates with other HHS departments, as needed, to resolve operational problems and make most effective use of the resources in delivering quality patient care. Will be required to rotate on-call throughout the calendar year.? Participates in Heartland's peer review process. Supports clinic compliance with all applicable federal, state, local, and HHS rules, regulations, protocols, and procedures governing the clinical provision of medical services as well as those relating to, but not limited to, workplace safety, public health, and confidentiality. Supports and is involved in HHS's continuous quality improvement efforts designed to improve patient outcomes. Works in consultation with clinical teams, direct clinical support staff, and indirect clinical support staff to develop and implement policies and procedures that maximize patient-centered communication and services. Maintains and assures confidentiality of patient information in accordance with HHS's policies. Reports building/equipment problems through the appropriate channels. Performs any clerical duty or department related task as assigned by supervisor in a continuously changing medical practice. Attends all staff meetings, clinician meetings, department meetings, and any other meetings as required. Requirements Master's degree in social work required. Fully licensed Mental Health Professional (LCSW, LCPC) Two years of clinical social work preferred. Clinical Social Work skills in a healthcare setting. Interdisciplinary teamwork skills in a health clinic setting. Some computer skills required. Excellent communication skills required. Electronic Medical Records (EMR) experience preferred but not required. Salary Description $62,000-$66,000 Annually
