Cumberland Heights Foundation offers a comprehensive benefits program, which includes: Medical, Dental and Vision effective 1st day of month following 28 days of employment Employer Contribution for Health Saving Account or Health Reimbursement Account 401K with Company match and eligibility after 90 days of employment Paid Time Off (PTO) accrual beginning day (1) one and up to 20 days of PTO the first year POSITION SUMMARY: The Post D/C Intensive Case Manager bridges both the responsibilities of a Recovery Care Advocate and those of an administrative assistant, focusing on long-term alumni engagement, case management, data collection, and clerical duties. This role involves working with multiple departments to ensure continuity of care post-discharge and requires schedule flexibility to meet the varying needs of alumni. The Post D/C Intensive Case Manager will develop and maintain relationships with alumni, providing ongoing support and assisting with administrative tasks critical to alumni programs. PRIMARY DUTIES AND RESPONSIBILITIES include the following: Meets face-to-face with the patient while in treatment for an initial orientation about the Alumni Program and its features. Builds trust and rapport with alumni through personal interactions presented with empathy, understanding, open dialogue, and supportive recovery principles. Coordinates long-term case management with a focus on continued support and alumni engagement, tracking progress and recovery milestones. Collects and manages data related to alumni outcomes, documenting relevant information in clinical software and other systems. Assists in clerical tasks, including data entry, report generation, and scheduling for alumni events, sessions, and follow-up appointments. Collaborates with multiple departments to ensure seamless alumni care, including communicating with healthcare providers, internal staff, and community resources. Facilitates telephonic, virtual, and in-person contacts with alumni, ensuring timely follow-up at designated intervals post-discharge. Assists with program administration, including coordinating events, workshops, and alumni gatherings. Supports administrative functions such as maintaining records, coordinating calendars, and ensuring proper distribution of information within the department. Works closely with the Alumni Services Coordinator to develop and implement program enhancements and improvements. Ensures confidentiality of all patient and alumni information in compliance with HIPAA and organizational policies. Maintains flexibility in schedule to support alumni needs, which may involve evening or weekend hours. Promotes a healing environment by providing guidance, resources, and continued encouragement throughout the alumni's recovery journey. Recommends process improvements to enhance the quality of alumni services and overall program effectiveness. Assists with managing program funds, ensuring proper handling and distribution where needed. SUPERVISORY RESPONSIBILITIES None Requirements High school diploma or GED required; Bachelor's degree preferred. CPRS or equivalent Required. Minimum of two (2) years of recovery experience, including current and active application of 12-step principles. Experience in administrative roles, including case management, clerical work, and data management, is highly preferred. Proficiency in Microsoft Office (Word, Excel, Outlook) and ability to learn clinical software programs. Strong interpersonal and communication skills, both written and verbal. Excellent organizational skills for managing multiple tasks and priorities. Familiarity with Twelve Step programs and philosophy, with the ability to teach and model the principles. Knowledge of and ability to recognize signs of intoxication/withdrawal and general recovery resources. Ability to work effectively with diverse populations, maintaining cultural sensitivity. Ability to deescalate crisis situations in a calm and supportive manner. A valid Tennessee driver's license in good standing required. WORK ENVIRONMENT Position is in an office setting; campus setting involves walking outside to various buildings and everyday risks or discomforts requiring normal safety precautions. Position requires frequent contact with people in crisis, including those who are emotionally volatile and may require additional safety precautions. May perform CPR/First Aid or Handle with Care as needed (being certified or eligible is required) Position is subject to varying shifts and/or long hours, along with some travel. Rotating on-call responsibilities may be required. Ability to lift up to 10lbs and/or push pull up to 20lbs; move around campus on uneven and rural terrain. Ability to speak, hear, see, sit, walk, stand, reach, and use fine/gross motor skills.
