Essential duties and responsibilities: Maintenance of patient Electronic Health Record (EHR): Enter laboratory results received from non-interfaced outside entities for provider review, attaching received document to order for quality assurance. Upon receipt of diagnostics, results link the received image to the correct order in the order module within chart. Ensuring the order is updated to "result received" and ready for provider signoff. Correctly categorize and differentiate documents into each individual electronic health record, ensuring that accurate patient information is input and maintained on all patients and that consistent record structure is maintained Upon notification from clinic or patient follow up on pertinent documentation not yet received for filing into patient's electronic health Work outstanding lab and diagnostics report. Identify performing facility. Pro-actively search for and obtain results from facilities, which have approved record access. Request and prepare any outstanding paper charts for electronic conversion Transfer of Protected Health Information (PHI): Receive and hold incoming patient record transfers initiated from outside health care facilities until patient care is established and chart is created Request patient record transfers from outside health care facilities, follow up on outstanding requests Prior to transferring patient records ensure accurate release of information is completed (verify patient identifiers, documents requested and entity releasing to) Update patient status and PCP as well deactivate all medications and orders per PHI clinical guidelines, for patients transferring care Obtain additional levels of approval for any nonstandard requests for release of information. Update and maintain electronic PHI log identifying patient information released Generate encounter for billing of release of information when appropriate, include with released documents. Maintenance of Protected Health Information: Adhere to Colorado State Law concerning maintaining and/or destruction of patient health information, to include public notification of destruction date. Purge existing paper medical records that meet destruction criteria. (annually) Assist with the orderly storage and filing of existing paper records pending destruction date. Prior to destruction of medical records, ensure patient does not an active record. Assist in locating needed charts. Prepare and convert to electronic format for filing into electronic record. Prepare information as requested for auditing purposes. Participates in any quarterly reviews and departmental staff meetings. Appropriate handling of incoming calls and messages ensuring needs are met in a timely fashion. Experience: Two years previous experience working in health/dental setting. Previous experience with electronic health record preferred.
