Description Position Title: YV LifeSet Case Specialist Supervisor Department: Transitional Teen Services (TTS) Reports to: Director of TTS Position Objective and Summary: The YV LifeSet Team Supervisor provides direct individual and group monitoring and supervision to a team of 5 YV LifeSet Case Specialists. YV LifeSet Team Supervisor facilitates and guides the case management process adhering to DCFS, YV LifeSet and UCAN policies and procedures. The YV LifeSet Team Supervisor provides case management supervisory services to their assigned team addressing the needs of young people ages 17.5 to 21 years of age. The YV LifeSet Team Supervisor will ensure the program model is delivered with fidelity on every case ensuring that all program standards and procedures are followed and met. The YV LifeSet Team Supervisor adheres to UCAN core clinical philosophy and mission. The LifeSet Team Supervisors guides and monitors case management services and the overall effectiveness of their YV LifeSet team. The YV LifeSet Team Supervisor functions in a leadership and teaching role with respect to the LifeSet model. The YV LifeSet Supervisor manages the day-to-day functions of the team and serve as the driving force of the team's engagement with the YV LifeSet program through trauma informed services and positive youth development. Essential Functions and Responsibilities: Assessment and Service Planning Ensures all YV LifeSet, DCFS, TPSN and UCAN case service plans, assessments for assigned clients are adequately and appropriately completed by each team member. Approves and provides sound feedback, when necessary, of client service plans, monitor service plan progress and reviews progress with team members, YV LifeSet clinical consultant, and DCFS as required. Ensures that all initial eligibility screenings with each young person is completed by utilizing the Pre-Enrollment Assessment (PEA) ANSA-T and Psychosocial assessments in a timely manner Assess and monitors the safety of clients in their current living arrangements by ensuring that the team is completing the necessary documentations in DCFS, and YV LifeSet data base systems. Oversees, the review and approval of the completion of the enrollment packet completed by the assigned YY LifeSet Case Specialist. Adheres to specific organization processes regarding the completion and review of other documents. Organization and Services Delivery Attends and co-leads weekly clinical consultations with YV LifeSet Clinical Consultant to review client service plans and assessments with the team providing feedback and gathering new information about the young adults. The LifeSet Team Supervisor is responsible for taking notes of recommendations, changes or ideas that are given by the clinical consultant. Meets for individual development with the clinical consultant once a week focusing on building supervisor skills in the role of Life Set Team Supervisor Documents quality and timeless of all service delivery standard, monitoring and if necessary, attending youth ACR, court and staffing's per case. Implements and develops YV LifeSet case specialist developmental plans through a collaborative approach with each case specialist reviewing goals that are developed and the steps that will be taken to accomplish them Attend all required DCFS, TPSN and LifeSet, TSS program meetings Serves as a support and liaison for the management team and the courts, DCFS monitor, other services providers, or community organization to coordinate client services or to make referrals Team Supervisor works with LifeSet Program Lead, Director on monthly data reviews and other necessary data required. Documentation and Reporting Responsible for ensuring documents are completed and of high quality by each team member related to the client's overall case. Makes frequent reports to Director of program on progress of client services and Life Set program model standards and procedures' Adhere to quality assurance program tracking and information gathering required by Life Set, DCFS and TPSN policies and procedures. Oversee client enrollment of necessary legal and intake documentation a of a young adult's enrollment into the program. Monitors teams accurate recording and completion of all client data i.e.: case note entry, as required by UCAN, DCFS and YV LifeSet Tracks all possible critical incidents and unusual incidents to director, program administrative, or DCFS as required by UCAN or by mandated reporting law. Review referral forms for behaviors, contact information and area of client concerns. Client Outreach and Engagement Attends and provide support to team during all meetings related to the young adults and/or family with the purpose of gathering information specific to intervention development and in the best interest of the young adult. Provides 24/7 on call services (rotating On-Call at night and on weekends) Communicates and provides guidance to team on appropriate youth services and engagement approaches to ensure quality case management services Services as a liaison between team members, community, and youth whenever necessary to support youth service plan and final living arrangement goals. Consults with YV LifeSet Clinical consultant as needed addressing any red flag issues or concerns. Support Parenting Skills and Services Monitors and ensure all TPSN& DCFS case management policies and procedures are adhered to for each pregnant and parenting youth in the program Assess, monitors, and supports staff on the safety of children living with clients and ensures that any needed recommendations and referrals for appropriate parenting services and placement is accurate and followed. Attends meetings and staffing with TPSN monitor on all pregnant and parenting cases when required. Education/Job Experience/Certification: MSW or MS/MA in Human Services field child-welfare, case management, counseling, psychology, or related field preferred Licensed Child Welfare Service Employee 3-5 years of case management supervisory experience Demonstrated understanding of trauma-informed practice Special Knowledge and Qualifications: Expected to adhere to all ethical and legal guidelines, including confidentiality and mandated reporting Ability to provide leadership and guidance to other staff members Proven ability to motivate and lead staff to achieve outcomes Ability to integrate UCAN's Clinical Philosophy into their role as Team Leader Is open and respectful of socioeconomic characteristics of clints and is willing and competent io work with a diverse client population Understands trauma informed practices Has and understanding of positive youth development Effective verbal and written communication skills Ability to work both independently and part of a high functioning team Experience or ability to use computer for data entry, communication and report writing Car and valid Illinois driver's licenses required. Must have CWEL.* Work Environment: This position requires local travel to TLP sites and communities when necessary. This position may require the transportation of clients and/or children. Work Conditions: This position requires after hours on call duties and may require work on weekend and evenings.
