OhioMeansJobs|Cleveland-Cuyahoga County is sourcing for Multiple Home Health Aid for Glory's United Services, LLC Essential Duties and Responsibilities: Assists the patient in activities of daily living including personal care, Hygiene, baths, back rubs, shampooing, skin and mail care, eating, dressing, eliminating, exercises, ambulation, and changing/making the patient's family to be involved in the patient's bed. Plan and prepare meals Encourage It helps in maintaining a safe and healthy environment. Use equipment and supplies safety and appropriately. Reminds the client to take, and assists the client and/or family, with self-administrated oral medications as directed by the physician, in compliance with laws in the state of operation. Takes and records accurate vital signs from the patient when advised. Follows the Home Health Plan as written, approved, and supervised by the Registered Nurse. Provides emotional support to the patient and/or family. Inform the Registered Nurse (RN) / Director of Nursing/ Registered Designated Nurse of changes in the patient's condition. Documents activities and findings and submits documentation as required by company procedures. Provide Home Hold services to improve patient's self-care at home. Document services performed or provided, as well as any communication with other health care providers. Take the necessary action in case of emergency and promptly report the case to the supervisor. Comply with agency policy and deadline, complete and submit daily progress notes, expense reports, timecards, scheduling records, and schedule changes. Ensure assignments are well organized, planned, prioritized, and completed following agency guidelines. Building relationships and trusting patients and their families. The Home Health Aide/Companions/STNA will not administer medications, take physician orders, or perform procedures that require the nurse's knowledge of training and skill, specifically sterile techniques. To perform this job successfully, a person must be able to perform each essential duty satisfactorily. You must have a high school diploma or general education degree (GED) or equivalent and meet training requirements in accordance with state and federal laws. Ability to communicate effectively with patient/client, family members, clinical management, and staff. Ability to read and write English, interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Write routine reports. Strong ability to be consistent and punctual to work. Good physical and mental health. Caring attitude, patience, respect, compassion, and good personal hygiene. Evidence that each homemaker meets one of the following qualifications: Successful completion of the nurse aide training and competency evaluation program conducted by the Ohio department of health under section 3721.31 of the Revised Code within the last 24 months. 1 year of supervised employment experience in a health or human services field and successful written and skill testing by return demonstration. Successful completion of the the Medicare Training and competency evaluation program for home health aides required under 42 C.R.F. part 484.4 and 484.36, if the person met those standards on or before January 12, 2018, and compliance with any additional requirements. Successful written and skill testing by return demonstration. Successful completion of at least 20 hours training and skill testing by return demonstration that includes the following topics: (universal precautions, meal preparation/nutrition, laundry, basic home safety, house cleaning, body mechanics, communication skills, emergenc
