Job Summary: CPSFL is s eeking a dynamic Targeted Case Manager ( Wraparound Facilitator) . The core function of a Wraparound Facilitator is to monitor, plan, link and advocate for their client and the client's family. A Wraparound Facilitator should be available to assist the families fold the needed services - including a full range of traditional and non-traditional mental health services, family support, health, education, and social services - into a family-sensitive service plan. Qualifications: A bachelor's degree from an accredited university or college, with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, or a related human services field. A minimum of one year of full time or equivalent experience working with children with a serious emotional disturbance. Have a Florida Driver's License and access to transportation. Bilingual a plus. SUPPORTING CLIENTS: Knowledge of the criteria by which one prioritizes service needs and objectives and can work with families to design service plans that address high priority needs. COMPLIANCE: Knowledge of and compliance with state and federal statutes, rules, and policies that affect the target population.
