Job Description SUMMARY OF DUTIES AND RESPONSIBILITIES A. Under the general administration of the Deputy Executive Director, the nurse practitioner is responsible for professional, clinical, educational, supervisory, and administrative activities in connection with the delivery of health care for patients within her/his particular scope of practice B. Under general supervision and in accordance with New York State requirements for written practice protocols and written agreements or for collaborative relationships with qualified physician(s) serves as a licensed, independent practitioner responsible for planning, coordinating, delivering and managing comprehensive health care services in a health care setting. Ensures patient and family-centered quality care to improve outcomes and patient experience, in accordance with applicable NYC Health + Hospitals policies and procedures, state and federal rules and regulations and nursing standards of care. General tasks and responsibilities will include: Patient Care 1. Provides continuous comprehensive medical and nursing care, including preventive and supportive health care services to patients and families. 2. Manages health problems and coordinates health care for various patient populations. 3. Identifies and diagnoses illnesses and physical conditions, and implements therapeutic and corrective measures. 4. Consistent with the established nursing practice, differentiates between normal and abnormal findings in the screening process to detect changes in health status that require physician's intervention. 5. Develops and maintains partnerships with community-based organizations and makes referrals to consulting physicians and/or other health care professionals and community agencies, as necessary 6. Evaluates and interprets patient data, in order to plan and implement appropriate nursing interventions. 7. Consults with physicians and other health care practitioners in the evaluation of health screening findings to make prospective decisions about treatment that will best support improved outcomes and patient satisfaction. 8. Formulates differential diagnosis and the plan of care based on medical history, physical exams, nursing assessment and other pertinent information. Identifies health and medical risks and needs. 9. Develops, implements, and evaluates patient plan of care. Determines effectiveness and modifies as per clinical standards and practice protocols, where needed, to achieve medical and health goals. 10. Orders, conducts, and interprets diagnostic and laboratory tests. Identifies and prescribes appropriate pharmacological and non-pharmacological interventions and therapies. 11. Manages patient care as outlined in practice protocols, and collaborate practice agreement and authorized by Medical Board. 12. Establishes medical diagnosis as well as nursing diagnosis as a Certified Nurse Practitioner of New York in accordance with Article 139, Section 6902.3. 13. Records and documents data necessary to maintain and coordinate health care. 14. Is a clinical/educational resource person for nursing personnel. Professional Practice 1. Participates in development, implementation and evaluation of interdisciplinary Performance Improvement (Pl) activities for specialty. 2. Seeks constructive feedback and takes action to achieve goals identified in performance appraisal. 3. Provides clinical and didactic instruction to professional staff, students and community groups and conducts Inservice educational programs. 4. Initiates and participates in studies and research related to nursing practitioner services, health care and allied services. 5. Participates in hospital/nurse practitioner committees. 6. Provides clinical supervision to Nurse Practitioners and students. 7. Participates with all staff to achieve zero defects goal for Hospital Governance 1. Considers factors related to safety, effectiveness and cost in the planning and provision of patient care 2. Acts as a liaison between nursing and professional staff assigned to specialty. 3. Utilizes the Just Culture Algorithm to manage risk and prevent adverse outcomes. 4. Participates in programs to improve communication, safety and interdisciplinary teamwork. Self-Development 1. Evaluates own practice in relation to professional standards, and regulatory requirements. 2. Maintains clinical competency by keeping abreast of current trends in health care, legislation, regulatory requirements and nursing practice. Patient Satisfaction 1. Anticipates patient's needs and within scope and authority, takes initiative to meet those needs, keeps patients and families informed about actions taken 2. Provides consistent and timely follow-through which ensures that all customer requests are addressed in a prompt and courteous manner 3. Makes every attempt to defuse potentially volatile situation 4. Resolves differences in a spirit of cooperation; creates solutions that benefit all parties 5. Able to maintain composure, efficiency and positive customer service-oriented manner during periods of peak workload 6. Participates in the Customer Satisfaction initiatives 7. Adheres to the ICARE standards of Customer Service Minimum Qualifications 1. A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED); and 2. Current certificate as a Nurse Practitioner issued by the NYSED; and 3. Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
