Job Summary Under the supervision of the Program Manager, the Intensive Field Capable Clinical Services (IFCCS) Rehabilitative Specialist provides individual rehabilitation and case management services for clients. The IFCCS Rehabilitative Specialist coordinates care with intra-agency staff, county workers, school personnel, referring agencies and the community at large. In addition, they serve a caseload of children and/or adolescents in the school, home, and community settings. The IFCCS Rehabilitative Specialist must also submit all required documentation in accordance with Aviva, DMH, DCFS, and COA standards. Essential Functions Consistently serve a caseload of children and/or adolescents with Individual Rehabilitation, Targeted Case Management, and Collateral services in the home, school, and community setting Complete and submit all documentation and Case Records Reviews in accordance with agency and Department of Mental Health (DMH) regulations including, but not limited to progress notes, client functional evaluations, outcome measures, CFT matrices, and treatment goals Available 24/7, responsible for responding and providing support during crisis situations Consistently meet agency billing productivity requirements and on-time submission of notes and other documentation Ability to build protective capacity with families and Core Practice Indicators (Engagement, teaming, assessment and understanding, long term view, planning, cultural competence, transitions, crisis management). As needed, provide community outreach to generate referrals including but not limited to presentations at schools, community organizations and functions, tabling at community fairs and outreach events, networking with community partners. Continue to provide services and treatment for clients even if change in residence relocates client to another service area Responsible for initial enrollments within contract indicated timeframes Meet weekly with supervisor and consult as necessary when clinically indicated (e.g., potential Suspected Child Abuse Reports, other crisis situations) Maintain effective working relationships, and coordinate care, with intra-agency staff, County workers, school personnel, referring agencies, and the community at large In collaboration with team members, create and maintain Plan of Care and Crisis Safety Plan for families Assess the need to request flex funds and communicate need to immediate supervisor, completing related documentation as needed or requested. Administers outcome measures for child and families Sensitive and responsive to the cultural differences among the agency's client population Participation in all agency required meetings and trainings Adheres to all applicable Federal and State regulations for confidentiality, HIPAA, mandated reporting of suspected child or elder abuse Actively participate in quality improvement projects The IFCCS Rehabilitative Specialist will take on special projects or additional duties as assigned by the Program Manager. Competency Requirements -Ability to speak, read, write and understand English -Ability to work effectively as a member of the treatment team -Ability to successfully work with families of diverse ethnic and cultural backgrounds -Ability to promptly and appropriately respond to a crisis situation -Must have excellent oral and written communication skills -Must have excellent interpersonal skills -Must have strong organizational and planning skills -Knowledge of social services system and community resources -Demonstrate skill in operating Microsoft Word, Outlook and Excel -Must demonstrate attention to detail -Must have excellent customer service skills and phone manner -Bilingual (English/Spanish) preferred Education and Experience -Bachelor's degree in related field preferred -Previous experience working in a social services agency in a similar capacity preferred Certifications/Licenses -Valid CA driver's license Additional Requirements -Tuberculosis clearance -Department of Justice/Fingerprint clearance -Satisfactory completion of health screening -Proof of legal eligibility to work in the United States -Satisfactory driving record Delegates Authority To N/A Physical Demands While performing the duties of an IFCCS Rehabilitative Specialist the employee is frequently required to walk, drive and use their fingers to manipulate a keyboard or writing instrument. The employee is occasionally required to sit, stand and reach. The employee is rarely required to run, kneel and lift objects heavier than 15 pounds. Work Environment The work environment is indoors and outdoors. The noise level varies from moderate to loud. The work environment is community-based and requires conducting home visits.
